This position has a $10,000 Sign On Bonus! Key Responsibilities The Home Health Registered Nurse is a trained professional nurse who administers skilled nursing care. May be required to perform admission, routine, respite, or continuous care visits and assess patients. Responsible for overall coordination of the care delivered to her/his patient caseload. This is accomplished by making regular visits to patients wherever they reside, communicating regularly with the Interdisciplinary Team and Medical Director, identifying patient, family, caregiver needs and providing those needs in accordance with the attending physician's orders and the IDG plan of care. This position requires the ability to build rapport and communicate effectively with patients, families, physicians, facilities, and other IDG team members. In this role you will be responsible for: Provides services requiring substantial and specialized nursing skill, in accordance with the plan of treatments signed by the physician and may make the initial evaluation visit to the client. Demonstrates competency in all skills required for the agency. Provides initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points. Evaluates and regularly re-evaluates the nursing needs of the client; initiates, develops, implements, and makes necessary revisions to the clients plan of care. Initiates diagnostic, preventative, and rehabilitative nursing procedures as appropriate to the clients care and safety. Makes referrals to other disciplines as indicated by the client's needs. Observes signs and symptoms and reports to the physician and/or appropriate health professional as often as needed, or upon changes in the client's condition. Teaches, supervises, and counsels the client and family regarding home nursing procedures and other care needs as appropriate to the client's condition. Coordinates the total plan of care and maintains continuity of care by liaising with other health professionals assigned to the same clients. Attends client care conferences. Initiates client care conferences for complex and/or multidisciplinary clients whenever needed. Assigns home health aide services to specific patients. Supervises home health aide in alignment with Medicare Conditions of Participation and assesses the client's need for unskilled care and revises plan as appropriate. Communicates changes to the plan of care to the aide, patient/caregiver, and physician. Supervises LPN and paraprofessionals providing services. Develops, prepares, and maintains individualized client care progress records with accuracy, timeliness, and according to policies. Submits accurate and timely documentation. Responsible for following all policies and procedures regarding service delivery, documentation, and care coordination with full compliance. This includes, but not limited to: client education, infection control, management of hazardous wastes, and age specific behaviors for clients. Participates in in-service programs and quality improvement initiatives. Plans for client discharge from services. Required Qualifications Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Current State License as a Registered Nurse Minimum 1 year of experience as a nurse in a clinical care setting Nursing skills as defined by accepted nursing standards Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles Demonstrates ability to handle multiple priorities, documentation requirements, and deadlines. Reliable transportation and valid automobile insurance coverage Desired Qualifications BSN preferred Home Health or hospice experience preferred Location Office Location: 24255 W 13 Mile Rd STE 210, Bingham Farms, MI 48025 Main Service Area:  South Lyon and surrounding area Hours 8:00 am – 5:00 pm, Monday through Friday Pay Range $75,000 — $85,000 USD Background Screening Optimal Care conducts a background screening upon acceptance of a contingent job offer.
