Additionally, this role will involve conducting care plan reviews and mentoring and supervising intern pharmacists. The Clinical Pharmacist will report directly to the Pharmacist in Charge.</span></p>\n<p><br></p>\n<p><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt; font-weight: bold\">Duties and Responsibilities</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• Supervise pharmacists and intern pharmacists including mentoring, coaching, and managing according to company policies.</span></p>\n<p><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• Communicate with medical professionals regarding patient status and response to medication, and when necessary to resolve patient issues.</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• Create, review, and complete reassessments per Pharmacy protocol including conducting routine patient profile monitoring.</span></p>\n<p><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• Review and update Care Plans prior to each medication dispensing; assist in reviewing and discontinuing therapies for clinical appropriateness; troubleshoot infusion-related problems; and provide patient counseling when required.</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• Create and review Proactive Risk Assessments.</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• Participate in surveys conducted by authorized inspection agencies.</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• Pursue continuing education appropriate to job responsibilities and license requirements.</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• Report any misconduct, suspicious or unethical activities to the Compliance Officer.</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• Represent the company in a professional and courteous manner in all job-related interactions.</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• Ensure ongoing compliance with all laws and regulations and accreditation standards upheld by the company.</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• Perform other duties as assigned by supervisor.</span></p>\n<p><br></p>\n<p><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt; font-weight: bold\">Education and Experience Requirements</span></p>\n<p><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• Clear and Active California Registered Pharmacist License.</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• B.S. in Pharmacy.</span></p>\n<p><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt; font-weight: bold\">Preferred Qualifications</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• PharmD.</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">• <span>Ig Certified Pharmacist (IgCP®), Certified Specialty Pharmacist (CSP).</span></span></p>\n<p><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt; font-weight: bold\">Physical Requirements</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">The physical demands described here are representative of those that must be met by an employee to successfully</span><br><span style=\"font-family: tahoma, arial, helvetica, sans-serif; font-size: 12pt\">perform the essential functions of this job.
