Description
Job Title: Community Care Coordinator
Department : Street Medicine
Reports To: Street Medicine Program Manager
Job Summary:
The Community Care Coordinator is a vital member of the Street Medicine Program, responsible for verifying eligibility, registering patients in the EPIC Electronic Medical Record (EMR) system, supporting the healthcare team during outreach activities, and providing case management services. This role helps connect individuals to essential healthcare and resources, acting as a liaison between the community and the care team to ensure access to necessary services.
Key Responsibilities:
Verify patient eligibility for services, ensuring all documentation is complete and accurate.
Register patients into the EPIC EMR system during outreach activities, maintaining secure and accurate data entry.
Serve as the first point of contact for individuals in encampments, shelters, and other community settings, fostering trust and engaging them in care.
Navigate encampments, shelters, and transitional housing sites to locate individuals needing medical or social services.
Provide logistical support to the team, including driving to outreach locations, setting up mobile units, and assisting with patient flow.
Collaborate with healthcare providers to develop care plans, assist with follow-ups, and connect patients to appropriate resources such as housing, food assistance, and social services.
Offer case management support, including scheduling appointments, tracking referrals, and providing follow-up care.
Use personal experiences and cultural knowledge to build rapport with patients, promoting engagement in care and trust in the program.
Maintain accurate records of patient encounters and services provided, ensuring compliance with program protocols and HIPAA regulations.
Support safety protocols for the team during outreach activities and assist in managing emergent situations as needed.
Qualifications:
High School Degree required and AA preferred.
Bilingual (English/Spanish) Required.
Lived experience or significant experience working with underserved populations.
Familiarity with community resources, public health programs, and social services in the area.
Strong interpersonal and communication skills with the ability to navigate complex situations effectively.
Valid driver’s license with a clean driving record and comfort driving in urban environments.
Passion for community health, patient support, and improving access to healthcare services.
Physical Requirements:
Ability to stand, walk, and move for extended periods while providing care in various environments, including encampments, shelters, and mobile units.
Capacity to lift, carry, and transport equipment or supplies weighing up to 30 pounds.
Comfort working outdoors in varying weather conditions and navigating uneven terrain.
Ability to bend, stoop, and kneel as needed during patient care or outreach activities.
Adequate manual dexterity to perform medical procedures, operate equipment, and manage electronic health records on portable devices.
Visual and auditory acuity to perform essential functions such as patient assessments and effective communication.
Capability to drive and sit for extended periods while traveling to outreach locations.
