The following will automatically disqualify you from this requirement:</span></strong></p><ul><li><strong><span style=\"font-size: 16px;\">Driver&#39;s who are under the age of 23;</span></strong></li><li><strong><span style=\"font-size: 16px;\">Driver&#39;s with any major convictions in the past 36 months (DUI, drag racing, open alcohol container, reckless driving, driving while suspended, etc.);</span></strong></li><li><strong><span style=\"font-size: 16px;\">Driver&#39;s with more than three violations in the past 12 months (speeding, failure to yield, illegal passing, etc.);</span></strong></li><li><strong><span style=\"font-size: 16px;\">Driver&#39;s with more than five violations in the past 24 months.</span></strong></li><li><strong><span style=\"font-size: 16px;\">Driver&#39;s who have not held a valid driver&#39;s license in the US or Canada for a minimum of three years.</span></strong></li></ul><p><br></p><p><br></p><p><br></p><p id=\"isPasted\"><strong>POSITION SUMMARY:&nbsp;</strong></p><p>The Direct Care Staff at the Reach For Your Potential Day Habilitation complete a variety of tasks throughout the day. &nbsp;Each staff member is responsible for all of their assigned group&rsquo;s needs and activities. &nbsp; Direct Care Staff are encouraged to work together to ensure the best services and supervision for each of the members at all times. &nbsp;The Direct Care Staff are what makes our agency work; they are expected to be dependable and care for the members with patience and attention to individual needs and wants.</p><p>&nbsp;</p><p><strong>ESSENTIAL JOB FUNCTIONS:</strong></p><ul><li>Treat members with dignity and respect at all times.</li><li>Maintains collaborative professional relationships with member(s), Program Director, Service Coordinator, Facilitator and RN to better serve members.</li><li>Facilitates the implementation of member(s) ISP&rsquo;s and helps members complete their goals.</li><li>Abides by Reach For Your Potential&rsquo;s policies and procedures with a positive attitude.</li><li>Attend community outings and activities as scheduled. Assist with all activities of daily living, including toileting, feeding, and hygiene needs. Provide on-going supports to members.</li><li>Helps member(s) to advocate for themselves by gathering information, reviewing, and analyzing all aspects of an issue or concern.</li><li>Identifies crisis situations, discusses the incident with staff and members, and provides coping mechanisms to members.</li><li>Keeps up to date knowledge of member&rsquo;s health, symptoms, and medications.</li><li>Maintains necessary training to best serve members. Assist in keeping the clean by completing both assigned and unassigned duties.</li><li>Documents cash tracking accurately and obtains receipts.</li><li>Completes documentation before the end of shift, including Edoc notes, goals, and log prompts.</li><li>Turns in time sheet when due and fills out accurately. Switch shifts using the appropriate document. Works scheduled shifts and arrives to shift on time. Abide by the time-off requests due date and filled out correctly.</li><li>Document passing medication appropriately and accurately as well as schedule II medications.</li><li>Attends scheduled staff meetings.</li><li>Treats co-staff and supervisors with dignity and respect by listening to what others have to say and receiving constructive criticism.</li><li>Communicates accurate and important information to direct supervisor. Communicates effectively with co-workers.</li><li>Drives team-related work and respects the ideas and opinions of team members.</li><li>Presents themselves in a professional manner.</li><li>Helps to welcome and train new staff.</li><li>Handles stressful situations and helps to problem solve. Displays an ability to learn rapidly and adapt quickly to changing situations.</li><li>Other duties as assigned.</li></ul><p><br></p><p><strong>JOB REQUIREMENTS:</strong></p><p><br></p><p><strong>EDUCATION/ EXPERIENCE: &nbsp; &nbsp;&nbsp;</strong></p><p>High School Degree or its equivalent. &nbsp;</p><p><br></p><p><strong>SKILLS: &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;</strong></p><p>Must be able to read, write and communicate effectively. &nbsp;Patience in working with adults with disabilities is necessary. &nbsp; Needs to have a caring attitude, and be caring and supportive to members. &nbsp;Must be able to lift at least 50 pounds and assist with daily programming needs as required. &nbsp;A good driving record is required.</p><p>&nbsp;</p><p><strong>SUPERVISORY RESPONSIBILITIES:</strong></p><p>None.</p><p><strong>&nbsp;</strong></p><p><strong>EQUIPMENT TO BE USED:</strong></p><p>Knowledge of how to operate typical household appliances, including stove, microwave, dishwasher, etc. is preferred. &nbsp;Training for specialized adaptive equipment for individual members will be provided. &nbsp; &nbsp;&nbsp;</p><p>&nbsp;</p><p><strong>TYPICAL PHYSICAL DEMANDS:</strong></p><p>It is preferred that the employee is able to lift at least 50 lbs.
