Additionally, our Community Health Initiatives focus on addressing health disparities, making care more accessible to underserved populations. At POD Health, we strive to make healthcare personal, proactive, and accessible for all.</p>\n<p> </p>\n<p><span style=\"font-weight: bold\">Position Title</span>: Community Outreach Manager</p>\n<p><span style=\"font-weight: bold\">Location:</span> Florida (Tampa, Lutz, Lakeland, Odessa)</p>\n<p> </p>\n<p><span style=\"font-weight: bold\">Position Summary:  </span>As a POD Health Community Outreach Manager, you will be responsible for driving growth and expansion of POD Health’s services through provider-based and community referral development. This role focuses on building strong relationships with medical offices, senior-focused organizations, and community partners to generate Start of Care (SOC) opportunities for POD Health. You will serve as a key ambassador for POD Health — educating partners on our services, identifying eligible patients, and ensuring a smooth referral-to-enrollment process in collaboration with internal teams.</p>\n<p> </p>\n<p><span style=\"font-weight: bold\">Key Responsibilities:</span></p>\n<ul>\n<li>Proactively identify and cultivate Medicare-eligible referral sources to grow POD Health’s patient census.</li>\n<li>Build and maintain relationships with, but not limited to: Primary care practices, Specialist offices, Community health organizations, Senior centers, and advocacy groups</li>\n<li>Educate referral partners on POD Health services, eligibility requirements, and patient benefits.</li>\n<li>Drive POD Start of Care (SOC) volume by consistently generating and following up on qualified referrals.</li>\n<li>Plan, coordinate, and host community and provider-focused events, including: Health education workshops, Enrollment events, Senior center presentations, Provider lunch-and-learns</li>\n<li>Represent POD Health at community events, health fairs, provider meetings, and networking opportunities.</li>\n<li>Collaborate closely with intake, clinical, and operations teams to ensure timely follow-up and successful SOC conversions.</li>\n<li>Track and document all outreach activity, referrals, and follow-ups accurately in Salesforce within required timelines.</li>\n<li>Support POD Health growth initiatives, pilot programs, and special outreach campaigns as assigned.</li>\n<li>Maintain compliance with Medicare marketing guidelines and organizational standards.</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Key Performance Indicators (KPIs)</span></p>\n<ul>\n<li>POD Start of Care (SOC) Production</li>\n<li>Referral Growth: Consistent generation of qualified Medicare referrals month-over-month</li>\n<li>Provider &amp; Community Outreach: Engage with new providers or community referral partners per month</li>\n<li>Strategic Partnerships: Establish new Medicare-focused referral partnerships per quarter</li>\n<li>CRM Accuracy &amp; Timeliness: 100% accurate Salesforce documentation of outreach or referral activity</li>\n<li>Conversion Rate: Maintain a referral-to-SOC conversion </li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Required Qualifications:<br></span></p>\n<ul>\n<li>Bilingual Spanish fluency required</li>\n<li>3+ years of experience in: Healthcare marketing, Provider outreach, Community-based business development, Medicare or value-based care preferred</li>\n<li>Strong understanding of Medicare populations and community healthcare dynamics</li>\n<li>Proven ability to build provider relationships and convert referrals into enrolled patients</li>\n<li>Excellent interpersonal, communication, and presentation skills</li>\n<li>Cultural competency and experience working with diverse communities</li>\n<li>Proficiency in Microsoft Office and Salesforce CRM (or comparable systems)</li>\n<li>Valid driver’s license and reliable transportation for local travel</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Benefits:</span></p>\n<ul>\n<li>Health, dental, and vision insurance</li>\n<li>401(k) with company match</li>\n<li>Paid Vacation, Company Holidays, Sick Leave, and Floating Holidays</li>\n<li>Professional development opportunities</li>\n<li>Mission-driven work in an innovative healthcare space</li>\n</ul>",
