At Hospice of the West, we look for dedicated professionals who share our belief that true hospice care extends beyond medical needs—it’s about bringing dignity, peace, and human connection to every life we touch.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li><strong>Deliver Compassionate Medical Care:</strong> Provide comprehensive medical care, palliate, and manage terminal illnesses, ensuring strict compliance with the state Nurse Practice Act. All actions are performed under the supervision of a collaborating physician.</li>\n <li><strong>Collaborate for Holistic Patient Care:</strong> Consult with the patient's attending physician, the hospice medical director, and other healthcare practitioners to assess eligibility for hospice care. Collaborate on ongoing care needs and medical management related to the patient's terminal condition.</li>\n <li><strong>Coordinate and Oversee Patient Care:</strong> Review clinical information and seamlessly coordinate and oversee patient care in collaboration with the hospice interdisciplinary team and other healthcare practitioners. Ensure a well-coordinated and patient-centric approach to care.</li>\n <li><strong>Educate and Counsel:</strong> Engage in patient and family education and counseling. Address preventative care, medical problems, psychological issues, and spiritual concerns. Work collaboratively with the interdisciplinary team to identify care goals and meet the total needs of patients and their families/caregivers.</li>\n <li><strong>Recommendation for Certification and Recertification:</strong> Provide valuable input to the Hospice Medical Director by making recommendations regarding patient eligibility for initial certification and recertification for hospice care.</li>\n <li><strong>Stay Informed and Compliant:</strong> Maintain up-to-date knowledge of current Medicare/Medicaid, state, and federal rules and regulations for hospice services. Ensure strict compliance to provide the highest quality of care.</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>Master's degree with a minimum of one (1) year hospice or palliative care experience as an RN or NP preferred.</li>\n <li>Holds current unencumbered license in the State of Practice as a Registered Nurse and Nurse Practitioner.</li>\n <li>Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)</li>\n <li>Excellent verbal and written communication abilities, articulating complex problems if they arise.</li>\n <li>Must be computer proficient in typing and various programs, including background in EMR.</li>\n <li>Possesses and maintains CPR certification if required by state.</li>\n <li>Current driver's license, vehicle insurance, and access to dependable transportation.</li>\n</ul>\n<p><strong>Per Diem Nurse Practitioners are paid $150 per visit.</strong></p>",
