This role is ideal for someone passionate about supporting individuals with substance use and mental health challenges in a structured outpatient setting.</p>\n<h3>What We’re Looking For</h3>\n<p>We are seeking candidates who bring both clinical skill and compassion to their work. We’re here to help you reclaim your life and build a future filled with hope.</p>\n<h2>Responsibilities</h2>\n<p><strong>ESSENTIAL FUNCTIONS:</strong></p>\n<ul>\n <li>Conduct individual counseling sessions and group or family sessions, under supervision, as scheduled and document as required.</li>\n <li>May co-facilitate assigned group or family sessions and assist with modifying problem behaviors.</li>\n <li>May be responsible for initial assessments, as well as follow up assessment for patients.</li>\n <li>Prepare individual treatment plan for each assigned patient.</li>\n <li>Ensure all documentation regarding patient care, treatment and incidents are completed timely and in a clear concise manner.</li>\n <li>Assist with modifying problem behaviors and may also provide counseling services to families of patients.</li>\n <li>Act as a liaison between referral sources and patient.</li>\n <li>Provide crisis intervention to patient, as needed.</li>\n <li>May plan for aftercare for assigned patient.</li>\n <li>May act as a liaison with insurance carriers, as requested.</li>\n <li>May provide case management duties for patient, ensuring individualized quality care.</li>\n <li>Evaluate the needs of the patient and determine if referrals to other programs or facilities is needed.</li>\n <li>Demonstrate a positive, empathetic and professional attitude towards customers always.  When patient needs are not met, acknowledge and work to resolve complaints.  Recognize that patient safety is a top priority.</li>\n</ul>\n<p> </p>\n<p><strong>OTHER FUNCTIONS:</strong></p>\n<p>· Perform other functions and tasks as assigned.</p>\n<h2>Qualifications</h2>\n<p><strong>EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:</strong></p>\n<ul>\n <li>High School diploma or equivalent required. Bachelor's degree in social or health services field from an accredited college or university preferred.
