    "description": "<h2>Overview</h2>\n<p><strong>NEW HIGHER WAGES - UP TO $22.14/HR </strong></p>\n<p> </p>\n<p>PACE of CNY is looking for compassionate and reliable individuals, </p>\n<p><strong>which have their own vehicle</strong>, to work at Day Center Facility in East Syracuse. </p>\n<p> </p>\n<p>This is a Temporary Position ending on December 2nd, 2026. </p>\n<p>Must be already certified as a Home Health Aide in NYS</p>\n<p>Monday- Friday. No Weekends Required</p>\n<p>This is a Day Shift Position</p>\n<p> </p>\n<p>  PACE CNY has been providing services since 1997 and is one of the only 123 PACE providers in 31 states. Work with our unique program and team of healthcare professionals, which offer a complete range of health-related services, all designed to keep participants living in their own community or home as long as possible. The PACE CNY model is a Managed Long-Term Care program which provides an alternative to nursing home placement.  The goal is to maximize the independence of the elderly and enrich their quality of life by optimizing their functional ability. This is achieved with the various services that are available to our PACE participants, including our very own PACE transportation system, 2 day centers that provide medical clinics and regularly scheduled primary care appointments, dining services, and social/recreational activities.  </p>\n<h2>Responsibilities</h2>\n<p>Job duties specific to this position:</p>\n<ul>\n <li>Provides quality participant care and assistance with ADL's, IADL's and any other tasks as directed by the RN.</li>\n <li>Functions appropriately within the New York State Scope of Tasks.</li>\n <li>Observes for changes in participants' physical or emotional health and communicates the observation(s) to the participant's Case Manager. Appropriately utilizing the aide communication form.</li>\n <li>Performs medication supervision/remind/assist utilizing the 6 rights. Reports concerns regarding medications to the Case Manager.</li>\n <li>Documents timely and accurately all services provided on appropriate forms and signs and submits paperwork according to policy.</li>\n <li>Assists with spontaneous activities with participants such as games, puzzles, movies, etc.</li>\n <li>Order supplies (i.e. incontinence products) and coordinates delivery of supplies to participants' homes.</li>\n <li>Ensures that proper supplies are in the home (ie. thickened liquids, incontinence supplies, creams, etc.).</li>\n <li>Completes required in-service/education programs for a minimum of 12 hours per year including Corporate and PACE reorientations and fire safety. Attends biannual competency fairs and monthly staff meetings.</li>\n <li>Performs other care related duties as assigned.</li>\n <li>Understands and promotes Person Centered Care.</li>\n <li>Demonstrates effective communication skills while working cooperatively with Home Care staff.</li>\n <li>Adheres to service excellence by developing and maintaining positive respectful relationships with all customers, internal and external, to include participants, families, team members, all staff throughout the organization and community partners; performs responsibilities according to the highest quality standards.</li>\n <li>Displays compliance oriented behavior in the workplace. Is responsible for promoting and fostering compliance in the workplace.
