We foster a family environment where patients are treated with compassion and honesty. We believe that addiction is a chronic relapsing brain disease. Our staff is passionate about helping other people. We offer Medication Assisted Treatment and behavioral health services. We believe there is life beyond addiction and strive to help our patients discover their purpose and community in a healthy environment. We are looking for people who are willing to work in a fast paced medical environment.</span></p>\n<p><span style=\"font-size: 12pt\"><br></span></p>\n<p><span style=\"font-size: 12pt\"><span style=\"font-family: Arial, sans-serif; font-weight: bold\">Summary of Position</span><span style=\"font-weight: bold\">:  </span></span></p>\n<p><span style=\"font-size: 12pt\">Responsible for patient treatment and services including intake, assessment and integrative summaries, treatment planning and updates, and transition / discharge planning and referrals according to state of Colorado and federal regulations and CARF clinical standards </span></p>\n<p><span style=\"font-size: 12pt\"> </span><br></p>\n<p><span style=\"font-size: 12pt\"><span style=\"font-family: Arial, sans-serif; font-weight: bold\">Role and Responsibilities:</span><br></span></p>\n<ol>\n<li><span style=\"font-size: 12pt\">Completes screening / intake for patient eligibility for an ASAM Level 1 outpatient treatment, and refers as appropriate</span></li>\n<li><span style=\"font-size: 12pt\">Orients new clients and ensures their understanding of the program, its’ guidelines and opioid agonist medication</span></li>\n<li><span style=\"font-size: 12pt\">Conducts addiction treatment services independently for individuals, families, and groups. </span></li>\n<li><span style=\"font-size: 12pt\">Conducts assessments and develops an integrative summary used in the development of the individual’s treatment plan.</span></li>\n<li><span style=\"font-size: 12pt\">Prepares individualized treatment plan consistent with assessment findings and refers patients to services as appropriate.</span></li>\n<li><span style=\"font-size: 12pt\">Re-assesses and completes treatment / concurrent reviews on patients and revises treatment plans as needed</span></li>\n<li><span style=\"font-size: 12pt\">Completes transition plans and discharge summaries for discharged patients</span></li>\n<li><span style=\"font-size: 12pt\">Conducts individual sessions in accordance with the treatment plan, to provide feedback, support or encouragement, address behaviors and attitudes, or family, social or personal problems</span></li>\n<li><span style=\"font-size: 12pt\">Ensures presence of a release of information to protect patient privacy and confidentiality when communicating and following up with stakeholders (i.e., family, primary care physicians, other providers) </span></li>\n<li><span style=\"font-size: 12pt\">Models fidelity Evidence Based Practices and counselor competencies, as required.</span></li>\n<li><span style=\"font-size: 12pt\">Ensures and promotes a patients' rights to privacy, confidentiality, and other legal rights. </span></li>\n<li><span style=\"font-size: 12pt\">Maintains timely documentation in patient records in accordance with CBHC policies, Colorado and federal (CSAT and HIPAA) regulations, and CARF standards</span></li>\n<li><span style=\"font-size: 12pt\">Assists in preparation of the program for CARF accreditation and the implementation of organizational and clinical standards.</span></li>\n<li><span style=\"font-size: 12pt\">Maintains a working knowledge of state and federal regulations, and the CARF Opioid Treatment Program standards</span></li>\n</ol>\n<p><br></p>\n<p><span style=\"font-size: 12pt\"><span style=\"font-weight: bold\">Hours</span>: </span></p>\n<p><span style=\"font-size: 12pt\">8 hour days, M-F - 6am-2pm for 4 days a week and 1 day a week from 8am-4pm.</span></p>\n<p><br></p>\n<p><span style=\"font-family: Arial, sans-serif; font-size: 12pt; font-weight: bold\">Education, License/Certifications and Experience</span></p>\n<ul>\n<li><span style=\"font-size: 12pt\">CAS Certification with bachelor’s degree or higher-level education.
