We embrace and encourage differences in age, race, ethnicity, gender identity and expression, physical and mental ability, sexual orientation, socio-economic status and other characteristics that make people unique. </p>\n<h2>Responsibilities</h2>\n<p><strong>Primary job responsibilities:</strong></p>\n<ul>\n <li>Participates in establishing monthly goals for enrollment     </li>\n <li>May responds to individual inquiries regarding Fallon HealthWeinberg, including but not limited to, phone calls, web site inquiries, home visits, hospital and nursing home visits </li>\n <li>Reviews the enrollment process and the Fallon Health Weinberg Care Member Handbook with potential members/families.</li>\n <li>Completes the UAS-NY, estimates whether individual meets clinical criteria for enrollment, and submits UAS-NY.</li>\n <li>Develops and communicates initial Care plan for new members into the MLTC </li>\n <li>Communicates with the Care Teams to provide information for enrollment decisions and provides ongoing communication with referral sources, potential enrollees/families during the intake process</li>\n <li>Coordinates and communicates necessary information about potential enrollees to the Program Director on an ongoing basis     </li>\n <li>Closely monitors progress of pre-enrollment activities and notifies the Program Director of potential issues  </li>\n <li>Provides referrals for individuals not appropriate for FHW and for those who choose not to enroll</li>\n <li>Participates in weekly intake meetings</li>\n <li>Identifies Quality Improvement issues to the Program Director and participates in the development and implementation of the FHW Quality Improvement Plan as directed</li>\n <li>Assures uniformity in data collection</li>\n <li>Prepares monthly reports regarding referrals and enrollments</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Education: </strong></p>\n<ul>\n <li>Degree in Nursing;  BSN preferable</li>\n</ul>\n<p><strong>License:</strong>  </p>\n<ul>\n <li>RN, current license to practice in New York</li>\n <li>NY drivers license<strong>     </strong></li>\n</ul>\n<p><strong>Certification:</strong>        <strong>     </strong></p>\n<p> </p>\n<p><strong>Experience:  </strong></p>\n<ul>\n <li> Minimum of 2 years experience working with the  elderly, preferably in a community setting  </li>\n <li> Knowledge of UAS-NY Assessment process desireable</li>\n <li> Knowledge of Medicaid eligibility requirements desireable</li>\n</ul>\n<p><em><strong>Pay Range Disclosure:</strong></em><em>In accordance with the Massachusetts Wage Transparency Act, the pay for this position is $85,000-$90,000 annually which reflects what we reasonably and in good faith expect to pay at the time of posting.
