Positions available include:</p>\n<ul>\n<li><span style=\"font-weight: bold\">Registered Nurse (RN)</span></li>\n<li><span style=\"font-weight: bold\">Licensed Vocational Nurse (LVN)</span></li>\n<li><span style=\"font-weight: bold\">Home Health Aide (HHA)</span></li>\n<li><span style=\"font-weight: bold\">Physical Therapist (PT)</span></li>\n<li><span style=\"font-weight: bold\">Physical Therapist Assistant (PTA)</span></li>\n<li><span style=\"font-weight: bold\">Occupational Therapist (OT)</span></li>\n<li><span style=\"font-weight: bold\">Certified Occupational Therapy Assistant (COTA)</span></li>\n<li><span style=\"font-weight: bold\">Speech Therapist (ST)</span></li>\n<li><span style=\"font-weight: bold\">Medical Social Worker (MSW)</span></li>\n</ul>\n<p><span style=\"font-weight: bold\">Responsibilities:</span></p>\n<ul>\n<li>Conduct thorough assessments and develop individualized care plans.</li>\n<li>Provide skilled nursing care, therapy services, or social work support according to prescribed treatment plans</li>\n<li>Educate patients and caregivers on disease prevention and management.</li>\n<li>Collaborate with interdisciplinary team members to ensure holistic patient care.</li>\n<li>Document all patient care services accurately and timely.<br></li>\n</ul>\n<ul>\n<li>Flexible time for home visit</li>\n<li>One on one with your patient<br></li>\n<li>Independent use of professional skills</li>\n<li>Continues education and professional growth</li>\n</ul>\n<p>Initial Requirements:</p>\n<ul>\n<li><span style=\"font-weight: bold\">Valid California state licensure or certification in your respective field (RN, LVN, PT, PTA, OT, COTA, ST, MSW).</span><br></li>\n<li><span style=\"font-weight: bold\">Current CPR certification</span></li>\n<li><span style=\"font-weight: bold\">Current CA driver's license plus proof of car insurance.</span></li>\n<li><span style=\"font-weight: bold\">Minimum one year ST clinical experience, (two+ years is more preferable)</span></li>\n<li>Computer/Internet experience (online charting to be used)</li>\n<li>Bi/Multi-lingual skills a plus (e.g. Spanish, Russian, Tagalog, etc.)</li>\n</ul>\n<ul></ul>\n<ul>\n<li>Ability to travel to patient homes across Solano County, Napa County, and Contra Costa County.</li>\n</ul>\n<p><span style=\"font-weight: bold\">Benefits:</span></p>\n<ul>\n<li>Competitive compensation package.</li>\n<li>Weekly pay</li>\n<li>Flexible scheduling options.</li>\n<li>Mileage reimbursement.</li>\n<li>Paid sick leave and Individual Retirement Account (IRA) opportunities are available for all employees</li>\n<li>Professional development support.</li>\n</ul>",
