Collaborates with ancillary departments to ensure timely and appropriate care.</p><p>&nbsp;</p><p>Think you’ve got what it takes?</p><p>&nbsp;</p><p><strong>Job Duties &amp; Responsibilities</strong></p><p>Responsible and accountable for the daily care coordination of patients within area of clinical subspecialty. Utilizes evidence-based nursing interventions during clinical visits, telephone encounters, and electronic communications according to state regulations, regulatory agency standards, and organizational policies and procedures.</p><p>Determines which standards of care and practice apply to the patient; coordinates and communicates this information to other health care team members, in order to meet patient needs.</p><p>Coordinates visits with physicians, ensures appropriate consultations in hospital by working with the coordinators.</p><p>Coordinates visits with diagnostic imaging department and appropriate sub-specialists.</p><p>Coordinates initial education of patients by phone.</p><p>Assists intake coordinator with initial appointment.</p><p>Assists with pre-approval of procedures through conversations with insurance verifiers and case managers if necessary.</p><p>Ensures appropriate patients are presented at weekly multi-disciplinary case conference for care plan design and care coordination.</p><p>Provides follow-up to referring physicians through written documentation based on recommendations for clinical management discussed at the weekly multidisciplinary conference.</p><p>Uses appropriate evidence-based assessment, complex interviewing techniques and instruments related to ambulatory care nursing in collecting pertinent patient, family, or population specific data.</p><p>Coordinates communication among the coordinators and other staff.</p><p>Helps coordinate new and ongoing clinical research studies including IRB enrollment and data collection by working with the research department and Quality and Outcomes.</p><p>Provides education concerning various aspects of care.</p><p>Coordinates nursing care of patients across various departments and subspecialty services by demonstrating commitment to team, flexibility, professional interpersonal, communication, critical thinking, and customer service skills.</p><p>Collects and maintains data on patient referrals and consults and effectively manages program.</p><p>Collaborates with other members of the team and other key stakeholders.</p><p>Ensures staffing needs are met.</p><p>&nbsp;</p><p>As part of our commitment to maintaining a safe and healthy workplace, all successful candidates will be required to undergo respiratory fit testing in compliance with occupational health and safety standards.</p><p><strong>Skills &amp; Requirements</strong></p><p>Bachelor's Degree in Nursing or Associate's Degree in nursing with acceptance or current enrollment in a BSN program and signed agreement (see comments below) or an associate’s degree in a related field accepted by the Texas Board of Nursing for the purposes of obtaining and maintaining an RN license or Technical diploma in nursing with acceptance or current enrollment in a BSN program and signed agreement (see comments below) or must be currently enrolled in a nursing bridge program working toward an MSN. Verified BSN-equivalency or a signed agreement is required</p><p>NEW HIRES AND TRANSFERS FROM TCP/TCU OR HP/HPC EFFECTIVE 4/1/19: New hires, current TCP/TCU employee, and HP/HPC employees who enter the role effective 4/1/19 require either (1) a bachelor's degree or (2) an associate degree or technical diploma with current enrollment in a BSN program and a signed agreement to obtain a BSN. TRANSFERS FROM TCH/TWS/TPG EFFECTIVE 4/1/19: Current &nbsp;employees already in RN-required jobs who enter the role effective 4/1/19 require either a bachelor's degree in Nursing, an associate degree in Nursing or an associate degree in a related field accepted by the Texas Board of Nursing for the purposes of obtaining and maintaining an RN license, or a Technical Diploma in Nursing.
