    "description": "<h2>Overview</h2>\n<p><strong>PURPOSE STATEMENT:</strong></p>\n<p>To provide assistance with activities of daily living, i.e., eating, meal planning, bathing, dressing, hygiene/toileting, etc.</p>\n<h2>Responsibilities</h2>\n<p><strong>ESSENTIAL FUNCTIONS:</strong></p>\n<ul>\n <li>Provide assistance with light housekeeping needs that are directly related to the health and well-being of the eligible individual, i.e., bed-making, dusting, vacuuming, etc.</li>\n <li>Provide assistance with accessing community resources/shopping/appointments.</li>\n <li>Accurately complete and submit Daily Activity Logs and Contact Summaries in a timely manner.</li>\n <li>Adhere to safe transportation procedures.</li>\n <li>Follow appropriate protocol for completion of I/A and Serious Incident reports.</li>\n <li>Ability to work a flexible schedule based on the family’s needs.</li>\n <li>Attend trainings/in-services as requested and adhere to requests for personnel update information in a timely manner.</li>\n <li>Adhere to Activity Plan and/or family instructions regarding personal care of the eligible individual.</li>\n <li>Keep supervisor abreast of pertinent information relevant to the provision of services.</li>\n <li>Follow all safety policies and adhere to all worker's compensation program guidelines.</li>\n</ul>\n<p> </p>\n<p><strong>OTHER FUNCTIONS: </strong></p>\n<ul>\n <li>Perform other functions and tasks as assigned.</li>\n</ul>\n<h2>Qualifications</h2>\n<p> </p>\n<p> </p>\n<p><strong>EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:</strong></p>\n<ul>\n <li>High School Diploma or equivalent required.
