Teamwork, communication, and creating a fun atmosphere for our employees is key in our ability to provide top level care. <br> </p>\n<p>First Choice Home Health &amp; Hospice is seeking to hire a Part-Time RN Case Manager to care for our Home Health &amp; Hospice patients in Washington County. Visit types include admit oasis, high &amp; low acuity skilled follow-up visits, wound care, patient assessments, and care team coordination. Work is performed directly in the homes of the patients (see below for more detailed duties). <br></p>\n<p><br></p>\n<p>Competitive Salary <br></p>\n<p><br></p>\n<p>You will love working at First Choice if:</p>\n<ul>\n<li><span>You have a fun and caring personality</span></li>\n<li>You work well with a team</li>\n<li>You excel at educating patients and caregivers</li>\n<li>You are eager and willing to continue building your clinical skills</li>\n<li>You put patient care first, and are focused on the best goals and outcomes for the patient</li>\n</ul>\n<p><br></p>\n<p><span>Job Requirements:</span></p>\n<p>Active RN license in good standing</p>\n<p>Valid Driver's license<br>Reliable transportation</p>\n<p><br></p>\n<p><span>More Detailed Duties &amp; Responsibilities:</span><br></p>\n<ul>\n<li>Complete general assessment of patient including:\n<ul>\n<li>head to toe assessment<br></li>\n<li>assessment of wounds</li>\n<li>safety evaluation</li>\n<li>develop plan of care</li>\n</ul>\n</li>\n<li>Make follow-up visits to patients including:\n<ul>\n<li>checking vitals and comparing them to individual thresholds</li>\n<li>taking blood and urine samples, and delivering them to lab</li>\n<li>dressing changes </li>\n<li>checks blood sugar and gives insulin to diabetic patients.</li>\n<li>educate patient and care givers on what can be done to improve status</li>\n</ul>\n</li>\n<li>Coordinate with care team\n<ul>\n<li>report findings to MD as appropriate</li>\n<li>create CNA care plan and provide oversight</li>\n<li>work with care givers and therapists to reach patient goals and outcomes</li>\n<li>keep family members informed as appropriate</li>\n<li>develop discharge plan with team members</li>\n<li>Attend team meetings</li>\n</ul>\n</li>\n</ul>\n<p><span><br>Percentage of Responsibilities:</span></p>\n<p>Direct clinical care – 80%<br>Documentation and coordination - 15%</p>\n<p>Attending meetings – 5%</p>",
