YOU MUST HAVE a minimum of two years in hospice marketing.  This experience is mandatory, please do not apply if you do not have the minimum experience.   As an empathetic Hospice Community Liaison, you will play a crucial role in fostering relationships with physicians and healthcare professionals to educate them and their patients about hospice care and the services provided by Pacific Hospice. You will serve as the primary point of contact, promoting the understanding and utilization of hospice services, while demonstrating empathy, compassion, and expertise in end-of-life care. Your ability to build rapport, effectively communicate, and provide comprehensive information will contribute to improving the quality of life for patients and their families.</span></p>\n<p><br></p>\n<p><span style=\"color: #374151; font-size: 16px; font-weight: bold;\">Responsibilities:</span></p>\n<ol>\n<li><span style=\"font-size: 16px;\">Relationship Building: Cultivate and maintain strong relationships with physicians and healthcare professionals, including nurses, social workers, and case managers, to ensure they are aware of and knowledgeable about Pacific Hospice's services.</span></li>\n<li><span style=\"font-size: 16px;\">Education and Training: Conduct regular meetings, presentations, and educational sessions with healthcare professionals to educate them about the philosophy, benefits, and scope of hospice care, and specifically highlight the services provided by Pacific Hospice.</span></li>\n<li><span style=\"font-size: 16px;\">Patient Advocacy: Collaborate with healthcare professionals to advocate for patients who may benefit from hospice care, ensuring their understanding of the referral process and the criteria for hospice eligibility.</span></li>\n<li><span style=\"font-size: 16px;\">Resource Management: Maintain an up-to-date understanding of hospice regulations, policies, and industry trends to effectively address questions, concerns, and challenges faced by healthcare professionals and provide accurate information and resources.</span></li>\n<li><span style=\"font-size: 16px;\">Communication: Serve as a liaison between healthcare professionals, patients, and Pacific Hospice's interdisciplinary team, ensuring effective communication, coordination, and collaboration to meet the needs and preferences of patients and their families.</span></li>\n<li><span style=\"font-size: 16px;\">Relationship Expansion: Actively identify and engage with new healthcare professionals, clinics, hospitals, and community organizations to expand partnerships and referral networks, contributing to the growth of Pacific Hospice's services.</span></li>\n<li><span style=\"font-size: 16px;\">Documentation and Reporting: Maintain accurate records of interactions, referrals, and educational activities, ensuring timely and detailed reporting to the management team.</span></li>\n</ol>\n<p><br></p>\n<p><span style=\"color: #374151; font-size: 16px; font-weight: bold;\">Qualifications:</span></p>\n<ul>\n<li><span style=\"font-size: 16px;\">Mandatory two years previous experience working in a hospice or palliative care setting. </span></li>\n<li><span style=\"font-size: 16px;\">Bachelor's degree in a healthcare-related field (e.g., nursing, social work, healthcare administration) preferred.</span><span style=\"font-size: 16px;\">.</span></li>\n<li><span style=\"font-size: 16px;\">Existing hospice referral relationships and strong networking abilities, with a proven track record of successfully engaging healthcare professionals.</span></li>\n<li><span style=\"font-size: 16px;\">Excellent interpersonal and communication skills, with the ability to empathize, actively listen, and effectively convey complex information to diverse audiences.</span></li>\n<li><span style=\"font-size: 16px;\">Knowledge of hospice regulations, industry standards, and current healthcare practices related to end-of-life care.</span></li>\n<li><span style=\"font-size: 16px;\">Highly organized, detail-oriented, and able to manage multiple priorities effectively.</span></li>\n<li><span style=\"font-size: 16px;\">Familiar with Axxess EHR system, MS Office programs and gmail, along with other communication platforms such as Slack. </span></li>\n<li><span style=\"font-size: 16px;\">Compassionate, caring, and able to maintain emotional resilience while working in sensitive end-of-life situations.</span></li>\n<li><span style=\"font-size: 16px;\">Valid driver's license and reliable transportation.</span><br></li>\n</ul>\n<p><span style=\"font-size: 16px;\">Pacific Hospice and Palliative Care offers Medical, Dental and Vision insurance, paid time off and other great benefits -  to find out more, apply today! </span></p>",
