Team Impact
Under general supervision of the Nurse Manager, the Triage Nurse performs care coordination and professional nursing functions to assess the severity of the patient’s health concerns using protocols and algorithms in a manner that provides patient dispositions to emergency care, urgent care, clinic provider or nurse appointments within the scope of the licensed practice. The RN will support providers in triaging all walk in patients.

Responsibilities
The triage nurse will be part of a care team supporting general care processes alongside provider staff.
Manage and address nurse directed patient telephone calls and from walk in patients requesting healthcare services.
Evaluates and assesses patient’s health care needs based on patient’s signs and symptoms utilizing established triage protocols and guidelines and clinical judgement.
Obtains appropriate input and direction from provider as needed to determine the best clinical course of action and recommendations for patient specific patient healthcare needs.
Accurately documents symptoms, concerns, nursing assessment, advice provided and the patient/caller response. Follows established protocols and procedures to ensure consistency and department effectiveness as well as to improve healthcare outcomes of patients and callers and their access to appropriate level of care.
Coordinate and/or schedule same day and walk in appointments on the provider schedule.
Utilize standing orders, triage protocols and algorithms appropriately.
Provide patient education: e.g. home care measures, education on chronic disease self-management, prevention and relay symptoms of when to seek emergency care.
Assures that pertinent medical information is forwarded to proper departments as needed for continuity of care.
Administers certain medications, injections, immunizations and family planning options as ordered by the provider.
Monitor patient flow in the health center; ensure that patients receive all recommended services in a timely manner.
Documents in electronic health record (EHR), triage assessment, plan, implementation, evaluation and follow up.
Manage prescription refills per protocol, policies and procedures.
Education/Accredited Programs
Minimum of one year of recent primary care or specialty care experience in an ambulatory setting.
Must have knowledge of disease processes, normal growth and development of all age groups.
Must possess community health knowledge and theory.
Must operate with cultural sensitivity with respect to working with persons and groups of diverse racial, ethnic, economic and social backgrounds.
General computer skills are required. Experience with EHR systems.
Licenses/Certifications/Registrations
Unrestricted NY Registered Nurse license.
Current CPR/BLS certification.
Skills/Abilities
Nursing assessment skills with/without visual inspection utilizing basic nursing theory.
Critical clinical thinking skills required.
Excellent communication skills, i.e. listening, oral and written English skills while conversing with patients, families, health professionals and community agencies.
Demonstrated clinical competence.
Capacity to function independently within licensed scope of practice.
Must possess ability to handle stressful emergency situations calmly and effectively.
Salary Range
The expected base rate for this Glens Falls, New York, United States-based position is $33.50 to $53.56 per hour. Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements.
