    "requisitionDescription": "<div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div>SAFY is seeking a <strong>full time Case Manager</strong> for our <strong>Cincinnati division in Ohio. The Cincinnati division serves families in Hamilton, Butler, Warren and Clermont counties.</strong><p>As a <strong>Case Manager,</strong> you will be<strong>&nbsp;</strong>responsible&nbsp;for the case management functions of a caseload of foster youth and homes. Works closely with youth, family, foster parents and community partners by equipping the youth and foster parents with the support and resources needed.</p><p><strong>&nbsp;</strong><strong>Who We Are</strong>&nbsp;</p><p>At SAFY, our mission is Preserving Families &amp; Securing Futures. We are a team of 500 employees across seven states providing services such as Family Preservation &amp; Reunification, Behavioral Health, Therapeutic Foster Care, Adoption Services and Older Youth Services so the families and children we serve can build resilience and thrive.&nbsp;</p><p>&nbsp;<strong>What You&rsquo;ll Do</strong>&nbsp;</p><ul><li>Travel daily to provide in-home, community-based services to families and youth in&nbsp;caseload.</li><li>Complete referral reviews and intake assignments. Communicate service availability to all assigned&nbsp;clients.</li><li>Provide instruction, intervention, and assistance as deemed necessary through the exercise of independent judgment and knowledge or as part of the treatment team to preserve the family&nbsp;unit.</li><li>Coordinate with county agency as appropriate and attend family sessions to promote successful clinical interventions with foster&nbsp;youth.</li><li>Build and maintain relationships with families and external case&nbsp;managers.</li><li>Coordinate and monitor service provisions for youth and family in the community and with school officials, courts and other appropriate parties involved with the client. This may include but is not limited to mental health, medical, educational, psychological, vocational and social&nbsp;services.</li><li>Develop and ensure completion of appropriate assessments including but not limited to Individualized Treatment&nbsp;Plans,&nbsp;Psychosocial&nbsp;Diagnostic&nbsp;Assessments&nbsp;(PSDs),&nbsp;and&nbsp;Mental&nbsp;Health&nbsp;Services&nbsp;Plans&nbsp;according&nbsp;to established time frames involving all appropriate parties as required by policy and regulations of the state and/or&nbsp;contract.</li><li>Timely document outcome of home visits, support sessions, safety assessments, interaction with parties involved&nbsp;in&nbsp;the&nbsp;youth&#39;s&nbsp;care&nbsp;and&nbsp;incident&nbsp;reports&nbsp;as&nbsp;required&nbsp;by&nbsp;policy,&nbsp;regulations,&nbsp;and&nbsp;contract&nbsp;requirements.</li><li>Ensure foster parents, youth and case manager needs are met during periods of staff&nbsp;transition.</li><li>May require rotating on-call&nbsp;schedule.</li><li>Participate in performance quality improvement process and execute any assigned&nbsp;tasks.</li><li>Successful completion of all required training pertaining to&nbsp;job.</li><li>Establish and maintain a positive working relationship, sensitive and responsive to the cultural differences with all levels of SAFY staff, clients, families and&nbsp;vendors.</li><li>Ability to work nontraditional hours daily/weekly to fulfill position duties; may include nights and/or&nbsp;weekends.</li><li>Ability to maintain confidential and sensitive&nbsp;information.</li><li>Other duties or special projects as&nbsp;assigned.</li></ul><p><strong>What You Bring</strong>&nbsp;</p><p>We are looking for people from diverse backgrounds and experiences who are inspired by our noble mission to make a difference in the lives of the children and families in our care.
