HART is composed of a team of three Intervention Specialists (Case Managers/Advocates), a therapist, Program Manager, and Program Director.&nbsp; We are a close team that learn from and grow with each other. We are committed to the youth we work with and their healing along with educating others around the important issue of CSEC.&nbsp;</span></span></p>\n\n<p><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\"><strong>POSITION SUMMARY:</strong></span></span><br />\n<span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Under the supervision of the Senior Program Manager, the CSEC Intervention Specialist will work to employ a survivor led CSEC model to help youth and young adults feel empowered to leave or reduce harmful situations, provide linkages to resources, co-facilitate CSEC prevention workshops for youth, and support in preparing and facilitating CSEC trainings to community partners and providers. The CSEC Intervention Specialist will also provide a coordinated response with San Francisco&rsquo;s Human Services Agency (SF HSA) according to the CSEC County Protocol. This includes participating in a rotating, scheduled on-call system to provide crisis response outside of regular business hours when assigned. On-call shifts may include evenings, weekends, or holidays, based on an agreed-upon schedule. The CSEC intervention specialist must be able to work with multidisciplinary teams and maintain strong working relationships with system providers such as: SF HSA, Juvenile probation, and SFUSD as they advocate for our clients and support system navigation. In addition, Intervention Specialists support the day to day office flow by greeting youth and other visitors, supporting navigation of the space, and access to basic needs.</span></span></p>\n\n<p><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\"><strong>ESSENTIAL FUNCTIONS:</strong></span></span></p>\n\n<p>&nbsp;</p>\n\n<ol>\n\t<li><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Provide case management in the community, at partner sites, and on site at the office to youth ages 11-24 and their families who are at risk of or are experiencing commercial sexual exploitation or engaging in survival sex.&nbsp;</span></span>\n\n\t<ol>\n\t\t<li><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">This includes following the client tiers of case management engagement as outlined in the program manual, with adjustments made on a case-by-case basis (typically meeting with clients once a week).&nbsp;</span></span></li>\n\t</ol>\n\t</li>\n\t<li><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Develop and update case plans for each active youth with SMARTIE (Specific, Measurable, Attainable, Realistic, Timely, Inclusive, and Equitable) goals.&nbsp;</span></span></li>\n\t<li><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Initiate contact with referral sources within 3 days of client assignment and outreach to youth and families/guardians within a week.&nbsp;</span></span></li>\n\t<li><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Provide direct support to youth and their families or support networks, including referrals and warm handoffs to appropriate community resources.</span></span></li>\n\t<li><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Conduct program intakes and assessments within the first three (3) meetings with each assigned client, in accordance with program guidelines.</span></span></li>\n\t<li><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Advocate and support for the youth when interfacing with the child welfare system (child welfare workers, placement staff) and other systems, including medical,juvenile justice, and schools.</span></span></li>\n\t<li><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Participate in CSEC case consultation meetings, such as CFTs</span></span></li>\n\t<li><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Support youth engagement and stabilization by providing practical assistance such as clothing, transportation, safety or housing supports, journals, food and grocery assistance, hygiene supplies, gift cards, and other necessary miscellaneous expenses.</span></span></li>\n\t<li><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Maintain ongoing caseload and electronic files, keeping accurate program data and complying with all program reporting requirements for periodic review.</span></span>\n\t<ol>\n\t\t<li><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Case notes must be completed weekly.</span></span></li>\n\t</ol>\n\t</li>\n\t<li><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Balance multiple tasks in response to ongoing and emergent youth needs, crisis response, and
