Join us in making a real difference in the health of our community.</em></p>\n<h2>Job Summary</h2>\n<p>The HARP Supervisor will be responsible for the supervision of HARP Care Manager operations within the HARP/ Health Home Division. The HARP Supervisor monitors the departmental phone queue to ensure quality of calls between the care manager, members and providers. The HARP Supervisor conducts new hire training and continued training for all clinical staff. The HARP Supervisor participates and interacts with all staff in a supportive role as it relates to care management and coordination daily operations. The HARP Supervisor will enhance communication and processes within the clinical and non-clinical areas within and between other internal operating departments, to ensure that all member and employee needs are met</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Provides guidance within the HARP Integrated Care Department, particularly as it pertains to new processes and workflows which support program operations</li>\n <li>Promotes and facilitates a multidisciplinary approach, supporting HARP coordinated care operations amongst disciplines</li>\n <li>Oversee clinical program training/retraining and creation and updating of departmental training tools/workflows and resources</li>\n <li>Adhere to clinical standards of care through collaboration with providers in order to ensure appropriate outcomes</li>\n <li>Practice and adhere to departmental and state guidelines in order to protect self, members, and organization</li>\n <li>Supervising crisis calls and escalating when necessary</li>\n <li>Maintains a tracking tool which logs unacceptable inconsistencies and errors observed during quality reviews of recorded calls via the queue and care management documentation, care planning, follow up and interventions</li>\n <li>Performs quarterly audits of staff’s chart documentation</li>\n <li>Works collaboratively with HARP management team to ensure program goals, projects and initiatives are implemented and meet departmental workflows and policy standards</li>\n <li>Supports Care Management staff as needed when management is in meetings and/or working on other program initiatives</li>\n <li>Follows best practice and clinical standards, and adheres to departmental and State guidelines</li>\n <li>Performs all other duties or actions as required</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>MSW/MA/MS Master’s Degree or equivalent required</li>\n <li>LMSW, LCSW, LMHC, LMFT, RN preferred</li>\n <li>Bilingual in Spanish preferred </li>\n <li>Two years’ experience working with Behavioral Health and/or Substance abuse required.</li>\n <li>Prior supervisory experience required</li>\n <li>Previous Managed Care experience required in Medical Management/HARP Operations</li>\n <li>Knowledge of the Collaborative Care Model</li>\n <li>Experience with chronic condition management </li>\n <li>Excellent written and oral communication skills required</li>\n <li>Ability to multi-task well while maintaining a positive “can do” attitude</li>\n <li>Demonstrated ability to manage large caseloads in a fast-paced environment while building and enhancing team productivity</li>\n <li>Demonstrated professionalism and leadership skills along with the ability to develop, direct and support staff</li>\n</ul>\n<p>Salary: $62,000-$72,000</p>\n<h2>Equal Opportunity Employer</h2>\n<p><em><strong>Essen Health care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population. </strong></em></p> Apply for this job onlineEmail this job to a friend Share on your newsfeed ",
