    "requisitionDescription": "<div><p data-pasted=\"true\">As a Home Health Physical Therapist, you&rsquo;ll practice at the top of your license while delivering one-on-one rehabilitative care directly in patients&rsquo; homes. In this role, you will assess, plan, organize, and implement individualized therapy programs that improve mobility, reduce pain, restore function, increase strength, and help patients maintain independence and quality of life.</p><p>You&rsquo;ll partner with patients, caregivers, and an interdisciplinary care team to create meaningful outcomes through compassionate, evidence-based care.</p><h2>As a Physical Therapist, you will:</h2><ul data-spread=\"false\"><li>Perform and document comprehensive initial evaluations to identify patient needs and establish a clinical diagnosis prior to intervention.</li><li>Develop, implement, and modify individualized treatment plans to maintain, improve, or restore physical functioning.</li><li>Deliver therapeutic interventions focused on reducing pain, improving mobility, preventing disability, and maximizing patient outcomes.</li><li>Evaluate patient response to treatment and adjust care plans throughout the episode of care to achieve optimal results.</li><li>Apply infection control and universal precaution standards during all patient interactions.</li><li>Maintain continuity and quality of patient care through timely, accurate documentation.</li><li>Communicate effectively with patients, families, physicians, and interdisciplinary team members.</li><li>Support and promote agency mission, values, and operational policies.</li><li>Complete all documentation within established company timelines and guidelines.</li></ul><h2>Qualifications:</h2><ul data-spread=\"false\"><li>Graduate of an accredited Physical Therapy program.</li><li>Current and unrestricted Physical Therapist license in the applicable state.</li><li>Minimum of one (1) year of Home Health experience preferred.</li><li>Current CPR certification.</li><li>Valid driver&rsquo;s license and reliable transportation with current auto insurance.</li><li>Experience with HomeCare HomeBase preferred.</li></ul><h2>Physical Demands:</h2><ul data-spread=\"false\"><li>Ability to stand, walk, sit, climb, crawl, squat, and kneel for extended periods.</li><li>Ability to lift, carry, push, and pull up to 100 lbs.</li></ul><p dir=\"ltr\" style=\"line-height:1.38;text-align: justify;background-color:#ffffff;margin-top:0pt;margin-bottom:0pt;\" id=\"isPasted\"><br></p><p><br></p></div>\n",
