Teach\\/assist in developing\\/maintaining ADL skills. Conduct initial and ongoing assessments. Monitor routine health care of all individuals. Provide\\/arrange for routine and emergency healthcare. Review all health-related incidents. Maintain health records according to standards and regulations. Provide management and administration of medication in conjunction with psychiatrist and\\/or NPP. Maintain\\/coordinate effective and on-going communication with other service providers. Participate in the development\\/documentation\\/review and update of care plan. Act as linkage to community services. Coordinate and conduct care planning activities for a designated case load of recipients. Utilize health information technology and electronic health record to coordinate care and communicate with providers. Act as medical resource consultant for team members. Promote health and wellness activities for recipients via individual or group sessions. Provide crisis intervention services as necessary. Provide transportation as needed to necessary services. Promote self-advocacy and personal empowerment. May be assigned to participate in rotating afterhours\\/weekend on call coverage.
