In tandem with the clinical manager and clinical nursing staff, the nurse case manager helps to oversee patient care, nursing case management, clinical documentation (OASIS), referral staff, and customer satisfaction with home health patient care services. The incumbent brings content expertise and knowledge surrounding the program elements, deliverables, and metrics.
