The CHW will provide direct companionship care in an assigned geographic area. The responsibilities include outreach, friendly visits, light household chores, personal errands, and transportation. This position is also responsible for documenting direct care visits of members in service, assuring compliance with program regulations, and managing relationships with multiple stakeholders in the community.",
