We provide services in a caring, professional environment through the teamwork of our medical staff and employees.</p><p><strong>How you’ll contribute</strong></p><p>A Case Manager RN who excels in this role:</p><ul style=\"list-style-type: disc;\"><li>Monitors and manages patient care to promote continuity, optimal outcomes, satisfaction, cost efficiency, and regulatory compliance.</li><li>Consults regularly with nursing staff and the multidisciplinary team to evaluate patient status and appropriateness of care, including admission, length of stay, transfers, and discharge.</li><li>Leads and coordinates discharge planning, including transfers to other facilities, referrals to community resources, and discharge education.</li><li>Monitors patient and family satisfaction; responds to questions and concerns from patients, families, and payors.</li><li>Performs chart reviews to identify and address issues related to service delivery, outcomes, compliance, cost, and reimbursement.</li><li>Communicates effectively with providers, payors, and ancillary services to remove barriers and ensure timely care progression.</li><li>Accurately documents case management activities, plans, and outcomes.</li><li>Participates in performance improvement and required continuing education.</li><li>Supports department operations by working alternate shifts/units as needed; may be assigned on‑call.</li><li>Performs other duties as assigned.</li></ul><p><strong>Why join us</strong></p><p>We believe that investing in our employees is the first step to providing excellent patient care.
