Title: HIV Patient Navigator (Special Populations) Department: Health Care Coordination Reports To: Director, Client Services, Health Care Coordination Services Location: 56 Bay Street, Staten Island, NY 10301 Position Status: Full Time FLSA Status: Non-Exempt Pay Rate & Benefits: The pay range for this role is generally $41,600-$46,600 commensurate with relevant experience and qualifications and in alignment with internal equity. POSITION SUMMARY: The HIV Care Coordination Program (CCR) addresses HIV healthcare disparities by facilitating access to care and other services through a client-centered, holistic, and comprehensive approach to meeting the needs of persons through team-based care management. The program goals include prompt linkage and ongoing retention in HIV care, optimal treatment adherence, viral suppression, and reduced HIV-related mortality. This is achieved through activities such as patient navigation, directly observed therapy, health education, and coordination with the medical team; the frequency of support is driven by the intensity of an individual’s need. The program is community-based and requires in-person activity. The Navigator for Special Populations specializes in engaging priority populations, including men who have sex with men (MSM), transgender individuals, and those experiencing mental health or substance use challenges. Candidates with lived experience, such as personal or community-based experience with HIV, behavioral health, or navigating systems as part of BIPOC or LGBTQ+ communities, are encouraged to apply. This perspective helps build trust, promote equity, and deliver culturally responsive care. The Navigator supports clients in achieving program goals such as linkage to care, retention, treatment adherence, and viral suppression through activities like patient navigation, directly observed therapy (DOT), health education, and coordination with medical providers. The role is community-based and requires in-person engagement. DUTIES & RESPONSIBILITIES: Accompany clients to medical appointments and community agencies for support and advocacy. Coordinate logistics for care and treatment needs including appointment reminders, transportation, and childcare arrangements. Provide individual health education to clients regarding curriculum topics as they relate to the patient’s life. Assess treatment adherence and addresses barriers as they arise, particularly with populations experiencing intensive challenges in program, medical appointments, and/or medication adherence. Reassess client needs including the Self-Management Assessments and program eligibility. Participate in the development of care planning activities. Conduct Directly Observed Therapy (DOT), with enhanced support for clients facing adherence challenges. Coordinate with service providers and share relevant feedback with the care team. Participate in case conferences, clinical and individual supervision. Complete required annual training hours to meet agency and funder standards. Maintain accurate documentation and data submissions, including client records in AWARDS. Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values Other duties as assigned.
