Description Position Summary: T he Telephone Triage RN re p orts to the C omplex Care N ur s e M a n a g e r a nd w i l l a l s o receive further d i r ec t i ons f rom t h e Medical Director. T he Telephone Triage R N w i l l b e a m e m b e r o f t he Complex Care Management t ea m , a n d w ork co l l a b o r ati v el y w i th other members of the healthcare team. The Telephone Triage RN is responsible for assessing, coordinating, and providing access to care for patients within the practice who are most at risk for health deterioration, sentinel events, and/or poor outcomes. The RN will collaborate with all Providers in all Clinical areas to manage patient risk factors. A Telephone Triage Nurse is a licensed nurse responsible for assessing patients quickly and accurately to determine the urgency of care needed. Telephone Triage nurses prioritize patients based on symptoms, risk factors, and clinical presentation, provide initial nursing interventions as appropriate, and direct patients to the correct level of care (ED, urgent care, clinic visit, telehealth advice, or home care). Pr i m a r y R e spo n sibiliti e s: 50% of Time is spent on Telephone Triage Perform rapid patient assessments by phone to identify severity of illness or injury Prioritize patients using triage protocols and clinical judgment Collect and document patient history, vital signs, symptoms, and relevant risk factors Identify red flags and escalate care immediately for emergent conditions Provide patient education and guidance on next steps (home care, follow-up, or emergency care) Coordinate care with providers, EMS, clinical staff, and ancillary departments Initiate Standing Orders or nursing interventions per policy (labs, EKGs, medications, imaging, PrEp) Triage in fast-paced environments with high patient volume Maintain accurate documentation in the EHR, including triage level, interventions, and communications Support infection prevention practices and isolation precautions when indicated Communicate clearly and professionally with patients and families, including de-escalation when needed Participate in quality improvement, safety reporting, and workflow optimization 40 % of Time is spent on managing In-basket messages and critical labs Co m p e te n t w i th all asp e c ts n o t e d in t h e job d es c r i ption f or a re gi s ter e d n ur s e. Ma n a g es in-basket messages and co-manages care with clinical providers. Qualifications Qual i fic a tio n s: Reg i s t ered N ur s e actively l i c e n s ed i n C o m m o n w e a l th of M a ssac h u s et t s ( B S N re q u i r ed ). Ex p e r i e n c e 1 to 3 years i n Ambulatory nurse triaging ( requ i re d) . Triage training or certification (preferred) Strong assessment skills and ability to use evidence-based triage protocols and clinical decision tools Ability to respond quickly in emergencies De t ail o r i e n te d , collaborative and able to work independently. De m o n s trat e s ex c e l le nt w r i tt e n v erb a l and interpersonal c o m m u ni c at i on. S e n s i t iv i t y to et h nic , cu l tur a l , g e n d er; b e l i e fs , a nd b e h av i or s . Current BLS certification (required). Strong customer service and conflict-resolution skills M u l t i - l i n g u a l i s pre f er r e d . Experience working with target populations including but not limited to MSM, MSM of color, IDU, Haitian-Americans and African immigrants.
