The team is multidisciplinary and is led by a LMHC, has 4 case managers, a peer specialist, a psychiatric provider, and a nurse -as well as counselors.&nbsp;The focus is removing barriers by accessing benefits, housing, medications medical services etc. and reduce hospital admissions.</p><p style=\"margin-left:0in;\">The hours are Monday - Friday, 8 AM - 5 PM.</p><p><strong>Overview:</strong></p><p>Primary duty is to provide skilled nursing services to mental health, substance abuse, or developmentally disabled clients, under the direction of a Registered Nurse or medical doctor, and to provide a variety of related services to maintain a safe, therapeutic environment. Services include but are not limited to assessment, treatment planning, direct service provision, person centered services using specialized knowledge and skills and management of client risk and safety. This position is a mixture of an office and community-based service delivery system.
