The RN hourly range is $32.08 to $47.38</strong>, and placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.</li>\n <li><strong><u>For Registered Practical Nurses (RPN) </u></strong>- This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. <strong>The RPN hourly range is $27.17 to $33.37</strong>, and placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.</li>\n</ul>\n<h2>What The Role Involves</h2>\n<ul>\n <li>Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Home and Community Care Support Services (HCCSS).</li>\n <li>Working with patients (<strong>both in-person</strong> <strong>and</strong> <strong>virtually</strong>) to provide exceptional nursing services</li>\n <li>Providing services in a variety of settings which may include in-home, clinic, retirement home or school</li>\n <li>Communicating and liaising with patients, family members and others within the interdisciplinary care team</li>\n <li>Performs other clinical duties as required</li>\n</ul>\n<p> </p>\n<h2>What You Bring</h2>\n<ul>\n <li>Registered Practical Nurse (RPN) diploma</li>\n <li>Current College of Nurses of Ontario (CNO) registration in good standing</li>\n <li>A strong desire and commitment to making a difference in the lives of our patients</li>\n <li>Exceptional interpersonal skills</li>\n <li>A valid driver’s license and vehicle as regular travel is required</li>\n <li>Excellent English verbal and written communication skills</li>\n <li>CPR Certification</li>\n <li>Current immunization record including 2 step TB test</li>\n <li>To protect our patients, we require a current vulnerable sector check and a clear background check</li>\n</ul>\n<p> </p>\n<h2>CarePartners In Your Community</h2>\n<p>In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings.
