    "requisitionDescription": "<div><div><p id=\"isPasted\"><strong>Summary: &nbsp;</strong></p><p>The Nurse (RN) is part of the Family Practice team that provides person-centered services addressing a breadth of beneficiary&rsquo;s needs, helping him or her achieve their personal goals.&nbsp;</p><p><strong>Essential Duties and Responsibilities:&nbsp;</strong></p><ul><li>Manages the medication system in conjunction with the practice providers, administers and documents medication treatment;&nbsp;</li><li>Screens and monitors the beneficiary for medical problems and side effects (including nursing assessments, completion of AIMS);&nbsp;</li><li>Build rapport and engagement with beneficiaries&nbsp;</li><li>Engage in health promotion, prevention and education activities;&nbsp;</li><li>If the beneficiary is in agreement, develop strategies to maximize taking medications as prescribed (e.g., reviewing home environment to find clues to remind beneficiary to take medications; work with beneficiary and whole person care provider to scale back the number of times medications are taking each day);&nbsp;</li><li>Communicating and coordinating services with other medical providers;</li><li>Educating the team in monitoring symptoms and medication side-effects;</li><li>Direct and supervise LPN staff;</li><li>Accurately documents all billable encounters into Southeastern Integrated Care&rsquo;s EMR (electronic medical record) system within 24 hours. Any corrections will be entered within 24 hours of being notified.</li><li>Other duties as assigned.</li></ul><p><strong>Supervisory Responsibilities:</strong></p><p>The Nurse (RN) position oversees Nurse (LPN) staff.&nbsp;</p><p><strong>Qualifications:</strong></p><p>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.</p><p><u>Education/Experience</u></p><ul><li>Minimum one year of experience working with adults with serious mental illness;</li><li>Associate&rsquo;s Degree of Nursing for (RN)&nbsp;</li></ul><p><u>Required Skills/Abilities</u></p><ul><li>Working knowledge of medications;</li><li>Ability to empathetically interact with beneficiaries and their families;</li><li>Strong clinical skills;</li><li>Operate independently or with minimal supervision.</li></ul><p><u>Certificates, Licenses, Registrations</u></p><ul><li>Valid NC Nursing License&nbsp;or Compact License that allows practice in NC;</li><li>Copy of a valid, unrestricted NC Driver&rsquo;s license, proof of insurance</li></ul><p><br></p></div></div>\n",
