    "description": "<p>EA Family Services is hiring a full-time Lead Care Manager for the Bakersfield area. This position would entail working with a dynamic team providing care coordination/case management for Medi-Cal enrollees. Must love working with people; have a respectful, and objective attitude; be friendly and outgoing. Must have a BA/BS in the social services field. Must be Bilingual, speaking both English and Spanish. The hourly pay is $23.00 per hour. Must pass DOJ/FBI/CAIC clearances.</p>\n<p><br></p>\n<p>Lead Care Manager responsibilities include:</p>\n<p>· Coordinating with individuals and/or entities to ensure a seamless experience for the member and nonduplication of services.</p>\n<p>· Engage eligible members.</p>\n<p>· Oversee provision of ECM and CS services, including the completion of the assessment, development of a care plan, and implementation of the care plan</p>\n<p>· Complete referrals on behalf of Members.</p>\n<p>· Offer services where the member lives, seeks care, or finds most easily accessible and within managed care plan (MCP) guidelines</p>\n<p>· Connect the member to other social services and supports they may need, including transportation and Community Supports</p>\n<p>· Advocate on behalf of members with healthcare professionals</p>\n<p>· Use motivational interviewing, trauma-informed care, and harm-reduction approaches</p>\n<p>· Coordinate with hospital staff on discharge plan</p>\n<p>· Accompany member to office visits, as needed and according to MCP guidelines</p>\n<p>· Monitor treatment adherence (including medication)</p>\n<p>· Provide health promotion and self-management training</p>\n<p>Lead Care Manager trainings include:</p>\n<p>· Enhanced Care Management and Community Supports Services</p>\n<p>· Methods and processes for data sharing</p>\n<p>· Member rights and responsibilities</p>\n<p>· Clinical protocols, evidence-based practice guidelines</p>\n<p>· Population of Focus for specific health needs and cultural awareness and sensitivity</p>\n<p>· Patient-centered care</p>\n<p><br></p>\n<p>Job Type: Full-time</p>\n<p>Salary: $23.00 per hour</p>\n<p><br></p>\n<p>Application Question(s):</p>\n<p>· Must pass DOJ/FBI/CAIC clearances</p>\n<p><br></p>\n<p>Education:</p>\n<p>· Bachelor's (Required)</p>\n<p><br></p>\n<p>Work Location: In person</p>\n<p>Job Types: Part-time</p>\n<p><br></p>\n<p>Schedule:</p>\n<p>· Monday to Friday</p>\n<p><br></p>\n<p>Work setting:</p>\n<p>· Hybrid remote</p>\n<p><br></p>\n<p>Ability to commute/relocate:</p>\n<p>· Varies: Reliably commute or planning to relocate before starting work (Required)</p>\n<p><br></p>\n<p>Experience:</p>\n<p>· Case management: 1 year (Preferred)</p>\n<p><br></p>\n<p>License/Certification:</p>\n<p>· CPR Certification (Required)</p>\n<p><br></p>\n<p>Willingness to travel:</p>\n<p>· 25% (Preferred)</p>\n<p><br></p>\n<p>Work Location: Varies</p>\n<p>Benefits:</p>\n<p><br></p>\n<ul>\n<li>Employee assistance program</li>\n</ul>\n<p><br></p>\n<p>Work Location: Hybrid remote in Bakersfield, CA 93301</p>",
