This role builds strong relationships with clients to help them stay engaged in medical care.<span> </span><span style=\"font-weight: bold\">Patient Care Navigators<span> </span></span>are committed to removing the client’s barriers to better healthcare resources and services within their area.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Harvest Healthcare Solutions</span><span> </span>offers a supportive environment whose employees work as a team to provide the highest quality of services to seniors.</p>\n<p><br></p>\n<p>This position is<span> </span><span style=\"font-weight: bold\"><em>Part-Time</em></span><span> (20 hours per week) </span>and requires community outreach within Santa Cruz county.</p>\n<p> </p>\n<p>Candidates MUST reside within the Santa Cruz county area. </p>\n<p><br></p>\n<p>Qualifications:</p>\n<ul>\n<li>Telephonic and field-based outreach to engage clients in our care management program. Prior work experience within the homelessness, domestic violence, or substance abuse communities as a resource for assistance.</li>\n<li>Ability to establish and maintain personal and professional boundaries while successfully providing supportive services.</li>\n<li>Establishes close relationships with partners and serves as a point of contact for patients</li>\n<li>Provides health education to patients to promote self-management.</li>\n<li>Communicates with Care Team members on a routine basis to support care delivery for patients.</li>\n<li>Identify and connect patients to resources for all clients to overcome barriers to care, such as transportation, housing, food, and other social service resources.</li>\n<li>Schedule and attend primary care physician appointments to review and update care plans with the Care Team.</li>\n<li>Ability to assess for and make appropriate referrals for any identified mental health or psychosocial problems.</li>\n<li>Ability to maintain client case records in a clear and concise manner in database.</li>\n<li>Provide comprehensive health navigation including assistance with:</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Physical Requirements:</span></p>\n<p><br></p>\n<p>Physical requirements are those that are present in normal office environment conditions and will involve light to moderate lifting. Duties may involve occasional evening and weekend work.</p>\n<p><br></p>\n<p>Job Type: Part-time (20 hours per week)</p>\n<p>Pay: $25.00 per hour</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Schedule:</span></p>\n<ul>\n<li>5-hour shift</li>\n<li>Monday to Thursday</li>\n</ul>\n<p><span style=\"font-weight: bold\">Education:</span></p>\n<ul>\n<li>Associates or bachelor’s in social or human services preferred</li>\n</ul>\n<p><span style=\"font-weight: bold\">License/Certification:</span></p>\n<ul>\n<li>Candidates must possess a valid California driver’s license and meet the State’s automobile insurability requirement.
