This position includes a mix of hybrid work with travel to Shared Living homes.&nbsp;</p><p><strong>About the Role:</strong></p><p>Shared Living Providers support individuals in their private home; they provide adequate supervision and support based on needs and desired goals as identified in their Person-Centered Plan. The Shared Living Coordinator provides direct oversight of contracted Shared Living Providers and plays a key role in ensuring high-quality, person-centered services for members participating in Shared Living.
