For over 40 years, Catholic Eldercare has been a vital part of the NE Minneapolis community meeting the needs of elders in a caring, dignified atmosphere. We are seeking staff who share these goals to join our team!</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Supervises and leads social work team.</li>\n <li>Responsible to complete Admission Paperwork including obtaining face sheet demographic information initially and ongoing.</li>\n <li>Identify the need for medically related social services and assist with the provision of these services.</li>\n <li>The Social Worker identifies the psycho-social and emotional needs of residents and assists with care planning</li>\n <li>Coordinate assigned resident case load and charting.</li>\n <li>Schedule, notify residents/representatives and participate in resident care conferences/special meetings with the multi-disciplinary team and provide necessary psycho-social information specific to each resident. Complete assigned MDS interviews. Complete social services portion of MDS Assessments as required.</li>\n <li>Maintain current knowledge of government regulations and community, public and private services directly related to the financial and psycho-social needs of residents (health insurance Medical Assistance Pending Medical Assistance, PAS, ACG programs, etc.)</li>\n <li>Respond to resident/family/staff concerns using discretion, confidentiality and judgment to involve all necessary persons and coordinate special care conferences when deemed necessary.</li>\n <li>Advocate for residents’ rights.</li>\n <li>Coordinate and update potential discharge plans for each resident.</li>\n <li>Perform all duties in a manner that is consistent with quality standards, infection control, and safety measures, which includes using Standard Precautions, at all times.</li>\n <li>Participate in and or may present job related in-service programs, new employee orientation, mandatory training, and attend meetings in the interest of improving skills, acquiring knowledge, and meeting state and federal regulations.</li>\n <li>Remain current on tuberculosis screening.</li>\n <li>Performs other related duties as assigned.</li>\n</ul>\n<h2>Qualifications</h2>\n<p>Required Qualifications:</p>\n<ul>\n <li>1-3 years Social Worker licensed by the Minnesota Board of Social Work.  </li>\n <li>1 year previous supervisory experience preferred. </li>\n <li>Ability to supervise newly licensed Social Workers. </li>\n <li>Ability to pass NetStudy 2.0 background screening.</li>\n <li>Experience working with individuals experiencing dementia and cognitive decline (preferred). </li>\n</ul>\n<p>Knowledge, Skills, and Abilities:• Effective Communicator and active listener who seeks to understand - in person, via electronic media, and in writing.• Social sensitivity and awareness of the values, practices, and reactions of others.• Service Orientation with a strong desire to help others and assumes their positive intent.• Positive, person-centered approach with resilience in dealing with difficult situations.• Demonstrated ability to effectively manage time and work priorities. • Organized, self-driven individual who functions well as part of a team.</p>\n<p>Job Context: Salary $70,720.00 to $81,120.00 per year based on experience and qualifications, Exempt (not eligible for overtime)In-person position, on-site, 100% time.</p>\n<p></p>\n<p> What Catholic Eldercare offers you:</p>\n<ul>\n <li>Competitive wages, credit for experience</li>\n <li>Benefits eligible - health, dental, vision, LTD, and life insurance!</li>\n <li>Located on bus line</li>\n <li>Scholarship for career advancement and to gain new skills</li>\n <li>Match for 403b contributions</li>\n <li>Paid time off and Holidays throughout the year</li>\n</ul>\n<p> </p>\n<p></p>\n<p><em>Catholic Eldercare is an equal opportunity employer.
