The general purpose of this position is to provide mental health services to youth and families referred to Northern Pines Mental Health Center.</span></span></span></p>\n\n<p>&nbsp;</p>\n\n<p><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"font-size:12px\"><strong><span style=\"background-color:#ffffff\">Essential Functions</span></strong><span style=\"background-color:#ffffff\"> include:</span></span></span></p>\n\n<ul>\n\t<li><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"font-size:12px\">Review and promptly respond to referrals. Clarify frequency and type of services referring worker is requesting</span></span></li>\n\t<li><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"font-size:12px\">Coordinate and conduct intake sessions to include: complete intake paperwork with the family, obtain billing information, authorizations and continued care requests as needed.&nbsp; This will be done in conjunction with the clinical supervisor</span></span></li>\n\t<li><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"font-size:12px\">Complete a treatment plan consistent with the needs identified in the evaluation, by the family and by the referring worker.&nbsp; Ensure the clinical supervisor signs the treatment plan</span></span></li>\n\t<li><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"font-size:12px\">Ensure all documentation is completed as mandated (current diagnostic assessments, ROI&rsquo;s and renewing ROI&rsquo;s, treatment plans, functional assessments, progress notes within 24 hours of service delivery)</span></span></li>\n\t<li><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"font-size:12px\">Prepare and provide content to clients in an individual and group setting</span></span></li>\n\t<li><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"font-size:12px\">Facilitate and co-facilitate client groups as agreed upon with the clinical supervisor and colleagues</span></span></li>\n\t<li><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"font-size:12px\">Maintain contact with the referral source as agreed upon&nbsp;&nbsp; </span></span></li>\n\t<li><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"font-size:12px\">Ensure a diagnostic assessment has been completed initially and annually thereafter</span></span></li>\n\t<li><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"font-size:12px\">Conduct transition planning and referrals as needed</span></span></li>\n\t<li><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"font-size:12px\">Participate in Rule 29 and CTSS Supervision and Consultation at which time progress notes are submitted to the supervisor</span></span></li>\n</ul>\n\n<p>&nbsp;</p>\n\n<p><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"font-size:12px\"><strong><span style=\"background-color:#ffffff\">Qualifications </span></strong><span style=\"background-color:#ffffff\">include:</span></span></span></p>\n\n<ul>\n\t<li><span style=\"font-size:12px\"><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"background-color:#ffffff\">Must qualify as&nbsp;a mental health practitioner as defined by the Minnesota Department of Human Services in one of the following ways: </span></span></span>\n\n\t<ul style=\"list-style-type:circle\">\n\t\t<li><span style=\"font-size:12px\"><span style=\"font-family:Verdana,Geneva,sans-serif\">Has at least 2,000 hours (approximately two years working full-time) of supervised work experience in the delivery of mental health services.&nbsp;OR</span></span></li>\n\t\t<li><span style=\"font-size:12px\"><span style=\"font-family:Verdana,Geneva,sans-serif\">Holds a master&#39;s or other graduate degree in behavioral sciences or related fields.&nbsp;OR</span></span></li>\n\t\t<li><span style=\"font-size:12px\"><span style=\"font-family:Verdana,Geneva,sans-serif\">Has completed at least 30 semester hours or 45 quarter hours of coursework in Behavioral Sciences or related fields AND has completed a practicum or internship that required direct interaction with adults or children served, and was focused on behavioral sciences or related fields.</span></span></li>\n\t</ul>\n\t</li>\n\t<li><span style=\"font-size:12px\"><span style=\"font-family:Verdana,Geneva,sans-serif\">Experience and training working with children.</span></span></li>\n\t<li><span style=\"font-size:12px\"><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"background-color:#ffffff\">A valid driver&rsquo;s license</span></span></span></li>\n</ul>\n\n<p>&nbsp;</p>\n\n<p><span style=\"font-family:Verdana,Geneva,sans-serif\"><span style=\"font-size:12px\"><strong><span style=\"background-color:#ffffff\">Compensation &amp;
