For details on how we handle your information, please review our Privacy Policy: https://21hhs.com/privacy-policy Conduct Start of Care (SOC) Admissions - OASIS Conduct follow ups, evaluation visits as needed in designated territory Make home visits to patients in designated territory Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures Monitor clients’ conditions, reporting changes to Clinical team Collaborate with physicians and agency staff to deliver the best care possible Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided. Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
