The Program Manager provides project management, performance improvement support, and strategic coordination across teams within Medicaid quality programs, including the Care Innovation and Community Improvement Program (CICIP), and the broader Population Health portfolio. This role drives program execution, fosters collaborative partnerships, and ensures high‑quality, evidence-based improvements aimed at advancing system-wide community and population health goals.

Responsibilities
Project & Portfolio Management
Coordinate projects within Medicaid quality portfolios, including:
Maternal and Infant Health, Access to Mental Health Services, Access to Treatment for Substance use Disorders, and Management of Chronic Health Conditions.
Independently develop, implement and manage multiple concurrent projects using standard project management principles, tools, and methodologies.
Communicate and reinforce clear roles, responsibilities, and expectations for project team members.
Ensure timely, high-quality deliverables and effective progression of project plans.
Performance Improvement & Change Facilitation
Facilitate utilization of the performance improvement methodologies by means of assisting project teams with the performance improvement process.
Collaborate with clinical providers, staff, and subject matter experts to identify, implement, and spread evidence-based practices.
Manage key operational and clinical metrics in collaboration with system data analysts to track progress and evaluate impact.
Lead and facilitate patient engagement activities to ensure patient perspectives and experiences shape project direction and outcomes.
Collaboration & Stakeholder Engagement
Connect the portfolio initiatives with internal and external partners to share our best practices and learn from other healthcare system successes.
Facilitate regular leadership and project team meetings with consistent cadence, clear agendas, and structured follow-up to accelerate improvement and remove barriers.
Support cross-functional stakeholder groups in operationalizing new, innovative initiatives and advancing strategic health improvement efforts.
Act as a liaison between portfolio initiatives and broader Population Health and Community Health department programs, ensuring alignment and visibility across the organization.

Qualifications
Minimum Required: Bachelor's Degree in Clinical, Engineering, Business, Public Health or a related field.
Formal improvement training preferred. Formal project management training preferred.
Minimum Required: 2-3 years’ experience in a related job discipline. Preferred: 1-3 years equivalent experience in healthcare.

"InternalResponsibilitiesStr": "<p><strong>Project & Portfolio Management</strong></p>\n<ul>\n <li>Coordinate projects within Medicaid quality portfolios, including:\n <ul>\n <li>Maternal and Infant Health, Access to Mental Health Services, Access to Treatment for Substance use Disorders, and Management of Chronic Health Conditions.</li>\n </ul></li>\n <li>Independently develop, implement and manage multiple concurrent projects using standard project management principles, tools, and methodologies.</li>\n <li>Communicate and reinforce clear roles, responsibilities, and expectations for project team members. </li>\n <li>Ensure timely, high-quality deliverables and effective progression of project plans.</li>\n</ul>\n<p><strong>Performance Improvement & Change Facilitation</strong></p>\n<ul>\n <li>Facilitate utilization of the performance improvement methodologies by means of assisting project teams with the performance improvement process.</li>\n <li>Collaborate with clinical providers, staff, and subject matter experts to identify, implement, and spread evidence-based practices.</li>\n <li>Manage key operational and clinical metrics in collaboration with system data analysts to track progress and evaluate impact.</li>\n <li>Lead and facilitate patient engagement activities to ensure patient perspectives and experiences shape project direction and outcomes.</li>\n</ul>\n<p><strong>Collaboration & Stakeholder Engagement</strong></p>\n<ul>\n <li>Connect the portfolio initiatives with internal and external partners to share our best practices and learn from other healthcare system successes.</li>\n <li>Facilitate regular leadership and project team meetings with consistent cadence, clear agendas, and structured follow-up to accelerate improvement and remove barriers.</li>\n <li>Support cross-functional stakeholder groups in operationalizing new, innovative initiatives and advancing strategic health improvement efforts.</li>\n <li>Act as a liaison between portfolio initiatives and broader Population Health and Community Health department programs, ensuring alignment and visibility across the organization.</li>\n</ul>\n<p><strong>Other Duties as Assigned</strong></p>",
