Seeks</p></li><li><p style=\"margin-bottom:0in;\">consultation as necessary.</p></li><li><p style=\"margin-bottom:0in;\">Monitors patient's responses to the treatment plan. Regularly communicates patient progress and possible goal revisions to the physical therapist.</p></li><li><p style=\"margin-bottom:0in;\">Documents services provided and patient response/progress.</p></li><li><p style=\"margin-bottom:0in;\">Educates the patient and family/caregiver about patient deficits.</p></li><li><p style=\"margin-bottom:0in;\">Assists with patient discharge planning.
