    "requisitionDescription": "<p style=\"margin-left:0in;\" data-pasted=\"true\"><strong>McGregor PACE&nbsp;</strong>(Program of All-Inclusive Care for the Elderly) is a community service program that provides in-home healthcare services to the elderly as an alternative to nursing home placement, allowing seniors to remain at home.</p><p style=\"margin-left:0in;\">We are looking for a <strong><em>Life Enrichment Coordinator</em></strong> who will be responsible for planning and implementing therapeutic recreational activity programs for participants.&nbsp;</p><p style=\"margin-left:0in;\"><strong>Responsibilities:</strong></p><ul type=\"disc\"><li style=\"margin-left:0in;\">Implements the coordinated, multi-faceted activity program designed by the Activities Director for frail, older adults, including those with mental illness or dementia, vision/hearing or physical impairments.</li><li style=\"margin-left:0in;\">With guidance from the Activities Director, will plan appropriate therapeutic activities and implement individual and group activities that accommodate the needs of the participants.</li><li style=\"margin-left:0in;\">Reports participant status changes to the Activities Director and other staff, as relevant.</li><li style=\"margin-left:0in;\">May be asked to perform initial and periodic participant assessments.</li><li style=\"margin-left:0in;\">Multidisciplinary team support includes completion of initial comprehensive assessments of participants by enrollment date: formal reassessments with reevaluation of the care plans that are to be completed as needed; assists in the coordination and documentation of a 24-hour care delivery for participants with input from all disciplines of the multidisciplinary team; initial and ongoing assessments of participants with a focus on physical, mental or emotional changes that are to be reported to the multidisciplinary team and reflected on the plan of care as needed.</li><li style=\"margin-left:0in;\">Communicates with participants and families regarding activity programming, calendar, and special events.</li><li style=\"margin-left:0in;\">Informs the Center Assistants and Volunteers of special activities.</li><li style=\"margin-left:0in;\">Each week, restock the Activity Cart/Activity Area with resources for independent leisure pursuits and small group activities that volunteers or Center Assistants can implement.</li><li style=\"margin-left:0in;\">Works with Activities Director to coordinate community involvement in activity programming (speakers, entertainers, etc.)</li><li style=\"margin-left:0in;\">Makes recommendations for purchasing activity supplies according to agency policy and procedure.</li><li style=\"margin-left:0in;\">Ensures compliance with PACE regulations.</li><li style=\"margin-left:0in;\">In collaboration with the Activities Director, assists with the coordination of other therapists, special programs as needed, including art, music, horticulture therapy.</li><li style=\"margin-left:0in;\">May be asked to assist with departmental and participant outcome data collection and analysis; participates in performance improvement initiatives, as relevant.</li><li style=\"margin-left:0in;\">Provides educational materials and professional presentations to agency staff, as relevant.</li></ul><p style=\"margin-left:0in;\"><strong>Qualifications:</strong></p><div style=\"margin-left:0in;\"><ul style=\"list-style-type: disc;\"><li style=\"margin-left:0in;\">High School Diploma required. A bachelor&#39;s degree in art, music, or recreational therapy is preferred.</li><li style=\"margin-left:0in;\">Certified Activity Coordinator preferred.</li><li style=\"margin-left:0in;\">Minimum two years of experience in a social, recreational or MultiCare facility providing and coordinating services for a frail or elderly population within the last five years, one of which was full-time in a patient activities program in a health care setting preferred.
