Become a part of our success story and experience firsthand why we're consistently voted the best place to work. <br><br><strong>Job Summary:</strong></p><p>Bloom Social Workers are cross trained to provide care in several different areas including but not limited to home visits, imminent and admission visits, as well as palliative care.  The hospice social worker provides psychosocial support to patients and families by providing skilled social work intervention to include, but not limited to providing resources, referrals, education, supportive counseling, discharge planning and assistance with final arrangements. This position requires driving, city wide. Care delivery primarily takes place in the patient's home, in a community, or in a community where the patient resides. Weekend availability required.</p><p><strong>Hospice Social Worker Responsibilities: </strong></p><ul><li>Develops and maintains therapeutic relationships.</li><li>Provides comprehensive psychosocial assessments.</li><li>Develops and maintains comprehensive plan of care. </li><li>Provides supportive counseling. </li><li>Facilitates smooth transition from care environments. </li><li>Mobilizes community resources. </li><li>Provides effective patient/family/caregiver teaching. </li><li>Supports the death/dying process. </li><li>Collaborates in providing patient care.</li><li>Creates timely and accurate documentation. </li><li>Assures patient safety.   </li><li>Utilizes resources effectively and efficiently.</li><li>Adheres to Bloom standards and continuously improves processes/services.</li><li>Maintains and enhances professional skills. </li><li>Adheres to high standards of personal and professional conduct.</li><li>50% of caseload home-based patients</li></ul><p><strong>Hospice Social Worker Minimum Qualifications:</strong></p><ul><li>Master's degree in social work from an accredited school and a minimum of 1-3 years of palliative care or hospice experience.</li><li>Must have demonstrated ability to assess emotional needs, as well as social needs of patients and their families.</li><li>Requires the ability to administer psychological/emotional assessment and define treatment planning from results.</li><li>Excellent communication and interpersonal skills.</li><li>Ability to effectively collaborate with interdisciplinary team in providing patient care. </li><li>Solution driven, creative and resourceful problem-solving skills.</li><li>Ability to function independently and effectively anticipate needs and problems. </li><li>Skilled in organizing and prioritizing work.</li><li>Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through.</li><li>Must possess professional image, positive attitude, enthusiasm and self-motivation.</li><li>Comfortable in maintaining professional boundaries. </li><li>Proficient computer skills.</li><li>Broad understanding of community resources including those related to financial benefits.</li><li>Awareness and sensitivity to cultural diversity.</li><li>Knowledge of self-care, personal boundaries; abilities, limits and inner resources.</li><li>Current CO Driver's license and automobile insurance.</li></ul><p><strong>Hospice Social Worker Preferred Qualifications:</strong></p><ul><li>Previous hospice/experience.</li><li>LCSW licensure.</li><li>Bilingual abilities a plus.</li></ul><strong>Why Bloom? </strong><ul><li>Competitive salary </li><li>Opportunities for professional growth. </li><li>Collaborative work environment with a passionate team making a difference in healthcare. </li><li>Employer-paid basic life and AD&D insurance.</li><li>Monthly car allowance</li></ul><p><em>Bloom Healthcare is an equal opportunity employer.
