Your role will be essential in facilitating difficult conversations while addressing fears, hopes, and wishes with sensitivity and understanding, helping patients and their families through this journey. By working closely with our interdisciplinary care team, you will ensure that every patient and family receives the necessary resources to maintain comfort and well-being.</p><p>We are seeking a NYS licensed social worker with a MSW degree. Committed to providing the highest quality of care for our patients and their families.</p><div><div><div><div><div><div><p><strong>This is a full-time position; the salary range for this position is &nbsp;$60k - $75k annually and is commensurate with education, experience and certifications.&nbsp;</strong><strong>This position is&nbsp;</strong><strong>benefits eligible and includes medical, dental and vision insurance, 401(k) retirement plan with profit sharing, mileage reimbursement, paid time off and other additional perks.</strong><strong>&nbsp;</strong><strong>You will primarily focus on providing care for patients and their families in Rockland County, NY, but will be called upon, from time to time, to provide care and comfort to our patients in Orange County, New York.</strong></p><p><strong>The position is Monday through Friday, (includes a weekend rotation on a monthly basis) and involves driving to the patient homes</strong></p><p><strong>Job Description Summary</strong><strong>:</strong></p><p><strong>Responsibilities of Social Worker will include, though not be limited to the following:</strong></p><ul type=\"disc\"><li>Contributes to patient and family counseling and education.</li><li>Participates&nbsp;in ongoing&nbsp;interdisciplinary comprehensive&nbsp;assessment.</li><li>Assesses&nbsp;patient/family psychosocial response&nbsp;to the terminal illness and&nbsp;evaluates coping skills.</li><li>Assists patients, family, and agency staff in utilizing community&nbsp;resources.</li><li>Assists team members in&nbsp;understanding&nbsp;the&nbsp;significant&nbsp;social and&nbsp;emotional factors related&nbsp;to the care&nbsp;of the&nbsp;terminally&nbsp;ill&nbsp;patient&nbsp;and&nbsp;family.</li><li>Coordinates with team members&nbsp;to plan and&nbsp;implement&nbsp;the hospice plan&nbsp;of care.</li><li>Social Worker will participate in interdisciplinary team meetings.&nbsp;</li><li>Social Worker maintains&nbsp;accurate, thorough&nbsp;and&nbsp;professional written&nbsp;records, and&nbsp;charts&nbsp;relevant&nbsp;information in&nbsp;a timely&nbsp;fashion,&nbsp;following&nbsp;UH documentation&nbsp;standards.</li><li>Participates in and&nbsp;is actively&nbsp;involved with&nbsp;the bereavement portion&nbsp;of the program as&nbsp;appropriate.</li><li>Participates&nbsp;in volunteer&nbsp;and staff&nbsp;education&nbsp;programs.</li><li>Develops and implements the discharge plan in conjunction with the IDS.</li><li>Participates in QAPI program and hospice sponsored in-service&nbsp;training.</li><li>Refers high risk&nbsp;patients.</li><li>Develops/maintains community resource&nbsp;file.</li><li>Incorporates organization&rsquo;s commitment to customer service and continuous quality improvement into daily work habits. &nbsp;This includes promoting positive co-worker and teamwork relationships and supporting the goals and mission as set forth by the agency.</li><li>Accepts additional responsibilities as assigned.</li></ul><p><strong>Qualifications:</strong></p><ul type=\"disc\"><li>MSW from a graduate school of social work is required. &nbsp;Must be licensed by the State of New York.</li><li>LMSW required, LCSW a plus. &nbsp;</li><li>Must have at least two years of supervised experience in the health field or two years of experience in family therapy/counseling.</li><li>Social Worker in this role must receive&nbsp;certification&nbsp;in hospice and&nbsp;palliative care&nbsp;within three years&nbsp;of hire date and maintain&nbsp;certification during&nbsp;employment.</li><li>Ability&nbsp;to&nbsp;relate&nbsp;to&nbsp;a&nbsp;diverse&nbsp;population&nbsp;and&nbsp;workforce&nbsp;in&nbsp;a&nbsp;personable&nbsp;and&nbsp;effective&nbsp;manner.</li><li>Ability&nbsp;to&nbsp;relate&nbsp;to&nbsp;the&nbsp;terminally&nbsp;ill&nbsp;and&nbsp;their&nbsp;families.
