Together, we are making a difference, one life at a time.</p>\n<p> </p>\n<p><strong>Your Mission in Action</strong></p>\n<p>The Clinic Medical Support Professional is responsible for providing South Plains area residents seeking non-emergency assistance with health screenings and basic health and wellness education, specializing in long-term diabetes care in accordance with Agency policies, licensing standards, and other federal, state, and local regulatory requirements<em>.</em> </p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Provide patients with routine health screenings, such as, but not limited to blood pressure checks, blood sugar tests via finger stick, and temperature checks</li>\n <li>Provide patients with basic health and wellness education for patients with chronic health conditions</li>\n <li>Provide health education to patients as it relates to promotion of proper health and wellness, nutrition disease prevention</li>\n <li>Initiate patient treatment plan based on prescribed health care needs and goals, physician orders, and health assessments</li>\n <li>Monitor and identify patient medical and behavioral changes and notify appropriate medical and/or behavioral health professional; document changes and responses using correct forms and documentation standards</li>\n <li>Ability to drive to community partner sites and work with the public</li>\n <li>Complete required documentation and record keeping based on licensing standards, Agency policies, and federal and state regulations</li>\n <li>Document and update patient charts/files with accurate and complete </li>\n <li>Work closely with Director and interdisciplinary team to provide the care that focuses on patients in need</li>\n <li>Work as team player and exhibit professional behavior and a positive attitude with both internal and external clients that reflects positively on the Agency and is consistent with Agency policies and practices</li>\n <li>Always develop and maintain positive working relationships with patients and interdisciplinary teams within established boundaries</li>\n <li>Work as team player and exhibit professional behavior and a positive attitude with both internal and external clients that reflects positively on the Agency and is consistent with Agency policies and practices</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Minimum Qualifications</strong></p>\n<ul>\n <li>Registered Nurse (RN) license in the state of Texas</li>\n <li>2 years’ direct patient care experience</li>\n <li>Valid Driver’s License</li>\n <li>Demonstrated ability to use clinical skills in a fast-paced environment with competing requests</li>\n <li>Ability to work under pressure using solid and appropriate decision-making skills</li>\n <li>Ability to work collaboratively with diverse groups of people</li>\n <li>Strong time management and organizational skills with the ability to complete assigned tasks during shift</li>\n <li>Strong interpersonal skills with the ability to effectively communicate with diverse people</li>\n <li>Ability to establish a positive rapport with clients </li>\n</ul>\n<p><strong>Preferred Qualifications</strong></p>\n<ul>\n <li>Bilingual in English and Spanish with the ability to fluently converse and write in Spanish (based on the population served)</li>\n</ul>\n<p><strong>Travel</strong></p>\n<ul>\n <li>This position may be required to travel up to 25%</li>\n</ul>\n<strong>Physical Demands & Work Conditions</strong>\n<ul>\n <li>This position requires sitting and looking and using a computer for long periods of time</li>\n <li>Position works within the community</li>\n <li>Position requires frequent standing and walking</li>\n <li>Lift, push, pull, move up to 25 lbs. </li>\n</ul>The physical requirements for this position with or without reasonable accommodation are representative of those that must be met by an employee to perform the essential functions of this position successfully.
