    "ExternalDescriptionStr": "<p><strong>Position Summary: </strong>The incumbent will ensure the delivery of high quality, safe home physical therapy in accordance with federal, state, professional and agency standards and requirement.\n</p><p><strong>Education, License &amp; Cert: </strong>Be a graduate of recognized school of physical therapy with a Bachelor’s of Science. Current licensure in the state of primary practice is required.\n</p><p><strong>Experience: </strong>Experience working with an adult and geriatric population is preferred.\n</p><p><strong>Essential Functions: </strong>1. To ensure the delivery of high quality, safe home physical therapy in accordance with federal, state, professional and agency standards and requirements.\n2. Independent judgement and excellent planning and problem‐solving skills are required.\n3. Communicates and demonstrate a professional image/attitude for patients, families, clients, coworkers, and others.\n4. Provide population appropriate therapeutic interventions resulting in positive outcomes as judged by quality monitoring and patient satisfaction.\n5. Supervision, including the appropriate documentation regarding the supervision of a Home Health Aide or a Physical Therapy Assistant.\n6. Document the course of patient care including progress made, continuing need for treatment and response to treatment.\n7. Provide patient and family education on tasks and resources that will assist with continued functional independence.\n8. Participating in continuing education/professional development activities.\n9. Participate in performance improvement management activities and committees sponsored by the agency and the Guthrie Healthcare System.\n10. Contribute to the delivery of cost‐effective care, maintaining visit and productivity requirements, as stipulated, by agency administration.\n</p><p><strong>Other Duties: </strong>1.Travel for this position is always required.\n2. It is understood that this description is not intended to be all inclusive and that other duties may be assigned as necessary in the performance of this position.</p><p>The contracted rate of pay for this per diem position is $68.00/hr</p><p>\n</p>",
