    "description": "<h2>Overview</h2>\n<p><strong>As a Clinical Liaison (Care Transitions Coordinator), you will assist patients with the process of navigating post-acute care.  Your goal is to create a positive impact on patient outcomes while increasing referral source satisfaction with Enhabit's services.</strong></p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Integrate evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered and promote quality and efficiency in the delivery of post-acute care.</li>\n <li>Represent the area home health branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities</li>\n</ul>\n<h2>Qualifications</h2>\n<p>Education and experience, essential</p>\n<ul>\n <li><strong>Must be a graduate of an approved school of nursing, therapy or social work. </strong></li>\n <li>Must be licensed in the state where they currently practice. </li>\n <li>Must have two (2) years of demonstrated field experience. </li>\n <li>Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice.</li>\n <li>Must have basic demonstrated technology skills, including operation of a mobile device.</li>\n</ul>\n<p>Education and experience, preferred</p>\n<ul>\n <li><strong>A Registered Nurse RN, Physical Therapist PT or Licensed Medical Social Worker LMSW is preferred.</strong></li>\n <li>Three (3) years of field experience is preferred.</li>\n <li>Previous experience in home health or healthcare sales is preferred.</li>\n</ul>\n<p>Requirements</p>\n<ul>\n <li>Must possess a valid state driver license</li>\n <li>Must maintain automobile liability insurance as required by law</li>\n <li>Must maintain dependable transportation in good working condition</li>\n <li>Must be able to safely drive an automobile in all types of weather conditions</li>\n</ul>\n<h2>Additional Information</h2>\n<p>As a national leader in home-based care, Enhabit is consistently ranked as one of the best places to work in the country.
