Description
Job Overview
This is a union position.
The Misdemeanor Incompetent to Stand Trial (MIST) Program serves defendants in Los Angeles that are facing misdemeanor charges and found incompetent to stand trial. This population is often chronically homeless with severe mental health disorders and, without alternative resources must remain in the County Jail, until their maximum sentence is served or are restored to competency. The Los Angeles’ Health Agency’s Office of Diversion and Reentry (ODR) has addressed the needs of this population through a diversion program that places them in residential community settings. Program participants are under an active Court order and required to remain medication compliant while in residence. Upon successful program completion, misdemeanor charges are dropped, and program participants are transitioned into permanent supportive housing. This particular HealthRIGHT360 (HR360) MIST facility is home to 19 male clients in a private, residential setting, that work with a multidisciplinary team comprised of: a Director, Clinical Supervisor, Mental Health Clinician, Case Managers, Nurse, Psychiatrist, Facility Monitors, and a Housing Manager. The LVN contributes to the HR360 multi-disciplinary team by supporting the clients with their medical needs.
The Licensed Vocational Nurse (LVN) reports directly to the Clinical Supervisor and works to maintain and support the medical needs of the clients. As part of the clinical team, the LVN will provide compassionate, client-centered clinical services to the clients while assessing and assisting with their medical treatment and services. The LVN will assist throughout the various stages of treatment including initial assessments and health screenings of medical needs, coordinating with different medical providers in the community, overseeing all medication monitoring and documentation related to medication, and discharge planning as it relates to knowing understanding when to take prescribed medications. The qualified candidate should have an active license and currently registered and in good standing with the BVNPT. The LVN will work to ensure that all medical documentation is completed while complying with all State, Federal and County documentation standards and HIPAA regulations.
Key Responsibilities
Treatment Responsibilities:
Provide basic nursing services as needed, including injections. Perform initial assessments, health screenings, and prepare client’s medical chart. Oversee medication monitoring and ensure proper documentation is accurate throughout all shifts. Remain current in knowledge of Medi-cal benefits and healthcare systems navigation. Input all client medication information into Welligent and Medisafe. Update client medications in Welligent and Medisafe as they change. Coordinate pharmacy services: order re-fills in a timely manner and track incoming medications as they arrive. Ensure medication inventory is accurate and pill count accurately reflected in Medi-safe. Interface with multidisciplinary team in treatment planning and service delivery, send a daily report outlining medication, daily audit discrepancies, and other relevant information regarding client’s health or changes in client’s care plan. Perform daily and weekly medication audits to ensure medication compliance. Serve as care coordinator for clients as assigned. Responsibilities include coordination of all services with other providers and completion of all coordinated care documentation. Coordinate PCP appointments, tracks result (including labs), and schedule follow-ups, as necessary. Deliver medications to clients when necessary. Maintain documentation in compliance with agency, HIPAA and DHS standards. Provide case management services to clients as assigned. Facilitate medication education and health-oriented groups with clients. Provide medication education consultations and collaboration with staff and student interns. Provide a “whatever it takes” approach to ensure clients receive the services they need. Assist in crisis and symptom management. Track vital signs and continuously assess for new onset of symptoms. Facilitate weekly COVID surveillance testing. Provide field-based services as required by program. Other duties as assigned.
Education and Knowledge, Skills and Abilities
Education Required:
High School diploma or equivalent. Licensed Vocational Nurse in California. Background Clearance Required:
Ability to clear and maintain a background clearance (must be off of Parole or Probation). Must receive Live Scan clearance. Skills and Abilities Required:
Some experience providing case management services required. Experience serving seriously and persistently mentally ill clients with co-occurring challenges. Knowledge of trauma preferred. Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data. Excellent verbal, written, and interpersonal skills. Integrity to handle sensitive information in a confidential manner. Action oriented. Strong problem-solving skills. Excellent organization skills and ability to multitask and juggle multiple priorities. Dependable automobile and insurance, registration and valid California Driver’s License. Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility. Strong initiative and enthusiasm and willingness to pitch in whenever needed. Desired:
Experience working with criminal justice population. Bilingual. We will consider for employment qualified applicants with arrest and conviction records.
