Community of Hope is seeking four full-time Housing Stability Case Manager for our Rapid Rehousing Program. In this role, you’ll help families secure and maintain stable housing, increase their self-sufficiency through income, benefits, and education, and strengthen overall family stability. Updates goal plans according to program contract guidelines and as needed, signed by client and case manager.&nbsp;</li><li>Meets with families on caseload to support implementation of their goal plans and follow-up in accordance with program contract guidelines and family needs.&nbsp;</li><li>Networks and coordinates with community resources, makes referrals as appropriate, and follows up to confirm and track families’ participation in housing, employment, medical care, substance abuse treatment, mental health treatment, training, etc.&nbsp;</li><li>Assists families with housing search process in collaboration with Housing Specialist. &nbsp;As needed, accompanies families to view units, advocates for families with landlords, and assists families with applications and other paperwork.
