While coordination of care is part of the job duties, the majority of the Behavioral Health Consultant’s activities will be focused on providing clinical interventions according to empirically supported treatment modalities.</span></span></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif\"><br></span></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-weight: bold\">Responsibilities and Duties:</span></p>\n<ul>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Provides on-site, telehealth and community behavioral health services in primary care clinic. </span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Maintains a schedule and presence that allows for immediate access by primary care providers. Promptly responds to provider calls and inquiries.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Provides initial assessment and/or behavioral intervention with patients referred by primary care provider(s)</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Focuses assessment and/or intervention on referral issue/question/concern.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Adheres to the principles of brief cognitive behavioral therapy and uses evidence-based or best practices when conducting behavioral interventions.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Directs interventions toward helping patients develop self-management skills.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Provides immediate feedback, verbal and/or written to primary care providers.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Conducts brief, episodic follow-up interventions on a short-term basis as needed. </span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Collects and utilizes data from screening tools for patient care.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Provides crisis intervention and evaluation services as needed.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Maintains current resource list and facilitates referrals as appropriate.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Studies current literature on behavioral interventions with medical clinic patients and learns new techniques that are approved by Evergreen Family Medicine for use in this project.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Assesses care needs of clinic population and works with providers, Clinic Administrator and Project Administrator to develop relevant program services (including involvement with support groups)</span></li>\n</ul>\n<p><span style=\"font-family: arial, helvetica, sans-serif\"><span style=\"color: black; font-weight: bold\">Qualifications and Skills:</span></span></p>\n<ul>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Master’s degree in psychology, social work, or counseling</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Two years in clinical service delivery with persons with mental or behavioral health needs. Previous experience working in primary care or other medical settings preferred.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Current licensure with the Oregon Board of Psychologist, Examiners (OBPE), or any one of the following licenses is acceptable for this position: Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT).
