    "requisitionDescription": "<div><div><div><div><div><div><div><p data-pasted=\"true\"><strong>Licensed Clinical Social Worker (LCSW)</strong></p><p><strong>Location- Dade City, FL</strong></p><p><strong>General Description</strong></p><p>Licensed Clinical Social Workers (LCSW) at Premier Community HealthCare Group (PCHG) provide patients with access to behavioral health counseling and essential community resources. LCSWs serve patients facing a range of challenges, including mental illness, abuse, addiction, family instability, and other personal issues. This role helps diagnose, treat, and manage acute and chronic behavioral health conditions while promoting mental wellness and strengthening community well-being. LCSWs collaborate closely with a multidisciplinary team to support the whole health of the patients served in our Dade City Location.&nbsp;</p><p><strong>Essential Duties &amp; Responsibilities</strong></p><p>&nbsp;&bull; Maintain a productive daily schedule<br>&nbsp;&bull; Conduct diagnostic evaluations, review medical histories, and provide therapy services<br>&nbsp;&bull; Communicate with other providers, school personnel, or agencies when needed for crisis intervention<br>&nbsp;&bull; Document patient encounters in the Electronic Health Record (EHR) within 48 hours<br>&nbsp;&bull; Review and address tasks daily; enter billing charges promptly<br>&nbsp;&bull; Educate patients on accessing community resources and navigating life with disabilities<br>&nbsp;&bull; Consult with fellow LCSWs and Behavioral Health leadership to ensure quality care<br>&nbsp;&bull; Provide counseling, diagnostic, and case management services to children, teens, and adults<br>&nbsp;&bull; Maintain confidentiality and uphold HIPAA requirements<br>&nbsp;&bull; Communicate changes that impact patient flow and site operations<br>&nbsp;&bull; Support community outreach initiatives and promote Premier&rsquo;s mission<br>&nbsp;&bull; Uphold Premier&rsquo;s core values and contribute to quality improvement (QI/QA) efforts<br>&nbsp;&bull; Perform other related duties as assigned</p><p><strong>Knowledge, Skills &amp; Abilities</strong></p><p>&bull; Understanding of clinical documentation and medical terminology<br>&nbsp;&bull; Strong communication&mdash;verbal, written, and clinical writing<br>&nbsp;&bull; Excellent interpersonal and customer service skills<br>&nbsp;&bull; Strong grammar, spelling, documentation accuracy, and organizational skills</p><p><strong>Qualifications</strong></p><p>&nbsp;&bull; Master of Social Work (MSW) from an accredited program<br>&nbsp;&bull; Current Florida Social Work License (LCSW)<br>&nbsp;&bull; Current Basic Life Support (BLS) certification<br>&nbsp;&bull; Two years of experience in counseling and resource coordination in community healthcare preferred<br>&nbsp;&bull; Ability to maintain Epic certification and annual compliance requirements</p><p><strong>Working Conditions &amp; Physical Requirements</strong></p><p>&nbsp;&bull; Lift 20 lbs. regularly and 30&ndash;50 lbs. occasionally<br>&nbsp;&bull;
