    "description": "<h2>Position Overview</h2>\n<ul>\n <li><strong>Department:</strong> Case Management</li>\n <li><strong>Schedule:</strong> Monday-Friday day shift, 5 (8 hour) Shifts per week</li>\n <li><strong>Work Location:</strong> UAB St Vincent's East</li>\n <li><strong>Benefits for eligible positions only include:</strong> generous paid time off, paid parental leave, Associate Assistance Program, Tuition Reimbursement Program, and more </li>\n</ul>\n<h2>What You Will Do</h2>\n<p>Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.</p>\n<ul>\n <li>Create plan for care across the continuum, integrating patient/family preferences and values.</li>\n <li>Monitor patient care through assessments, evaluations and/or patient records.</li>\n <li>Advocate for resources and removal of barriers.</li>\n <li>Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.</li>\n <li>Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.</li>\n</ul>\n<h2>What You Will Need</h2>\n<p><strong>Licenses / Certifications / Registrations: </strong></p>\n<p> </p>\n<p>Currently a Licensed Bachelor Social Worker (LBSW) or Licensed Master Social Worker (LMSW) by the Alabama State Board of Social Work Examiners required.</p>\n<p> </p>\n<p><strong>Education</strong>:</p>\n<p> </p>\n<p>Bachelor's degree in Social Work required.</p>\n<h2>Additional Preferences</h2>\n<p>No additional preferences.</p>\n<h2>About UAB St. Vincent’s</h2>\n<p>UAB St. Vincent’s, a proud part of UAB Medicine, is a trusted provider of health care, serving Alabama for more than 125 years.
