The Client Program Specialist develops a positive professional rapport with clients, communicates with outside agencies, completes initial and ongoing assessments, and conducts a collaborative treatment planning process. This position operates under a trauma-informed care framework, utilizes evidence-based practices in all client interactions, facilitates behavioral interventions and program reviews. &nbsp;&nbsp;</p><p>ESSENTIAL DUTIES AND RESPONSIBILITIES</p><ul><li>Ensures compliance with standard internal and external contracting agencies policies and procedures.</li><li>Conducts and documents effective and accurate client assessments and interviews to determine individual clients&rsquo; needs.</li><li>Collaborates with clients, referring agencies, and treatment providers to develop and implement case plans for client&rsquo;s treatment programs.</li><li>Evaluates, directs and monitors services and clients&#39; progress in relation to progression matrix and criminogenic needs with the use of evidence-based practices. Documents progression in internal electronic charting system.</li><li>Conducts client&rsquo;s initial orientation, SOA-R determined referral services, accurate and clear documentation in each client&rsquo;s paper and electronic file, treatment and work schedules, and releases.</li><li>Updates the collaborative treatment plan within regulatory timeframes and based on the client&rsquo;s unique strengths and needs.</li><li>Conducts and follows up on quality, evidence-based cognitive behavioral interventions.</li><li>Maintains a client caseload and meets with the client at least once a week to monitor progress in the program.</li><li>Communicates effectively and efficiently with courts, attorneys, probation officers, and parole officers regarding the progress of the client. &nbsp;Provides accurate and clear documentation as scheduled and upon request to all outside agencies.</li><li>Maintains professional relationships with clients from a trauma-informed care perspective to cultivate client engagement and accountability.</li><li>Creates various reports including but not limited to entry, termination, program reviews and client budgets.&nbsp;</li><li>Monitors compliance with clients&rsquo; financial obligations.&nbsp;</li><li>Begins discharge planning with clients at the time of admission to ensure stability in housing, finances, and employment.</li><li>Verifies all client employment and updates electronic records upon changes in employers.</li><li>Participates in cross-training for all Client Program positions.</li><li>Completes all required services and documentation within Embrave and regulatory guidelines.</li><li>Promotes and demonstrates the Mission, Vision, and Core Values of Embrave.</li><li>Develops and maintains good working relationships with all departments, the management team, regulatory agencies, and community partners.</li><li>Embraces cultural diversity amongst ourselves and our community.</li><li>Responsible for the human, financial, and material resources as well as data and information entrusted to us.</li><li>Performs other job-related duties as required.</li></ul><p><br></p><p>SUPERVISION EXERCISED: &nbsp;None</p><p>EDUCATION/ WORK EXPERIENCE:</p><p>Bachelor&rsquo;s degree in behavioral science or a related field required.
