Description

Are you a compassionate and organized professional with a passion for helping children and families thrive? AMHC is seeking a Children’s Community Case Manager to join our Madawaska team on a full-time basis. In this key role, you’ll coordinate care and support services for children with behavioral health needs, working closely with families, treatment providers, and community partners. You’ll lead care planning, advocate for clients, and ensure families get the services they need—all within a supportive, collaborative environment. What You’ll Do: Build strong, trusting relationships with children and families, even in challenging engagement situations. Develop and update individualized treatment plans in collaboration with families and providers. Coordinate and monitor services in line with treatment goals. Facilitate quarterly treatment team meetings and lead multidisciplinary collaboration. Advocate for families in schools, social services, and healthcare settings. Educate families about available behavioral health resources and support systems. Respond to crises and support the development of crisis and safety plans. Maintain timely, accurate documentation and ensure compliance with program and regulatory standards. Why Join Us?
